Combination of Upfront Systemic Therapy and Radiation in Advanced Non-Melanoma Skin Cancer

Checkpoint inhibitors and hedgehog inhibitors have shown promising results for treatment of locally advanced cutaneous squamous cell carcinoma (cSCC), Merkel cell carcinoma (MCC), and basal cell carcinoma (BCC) in the neoadjuvant setting. Given high clinical response rates to systemic therapy, we evaluated the role of upfront systemic therapy followed by consolidative radiation as a potential treatment strategy for those anticipated to suffer significant surgically-related morbidity. Herein, we describe our experience using upfront systemic therapy in combination with radiation in advanced non-melanoma skin cancer (NMSC) with 100% complete clinical response.
