Coronary and Extra-coronary Subclinical Atherosclerosis to Guide Lipid-Lowering Therapy
AbstractPurpose of ReviewTo discuss and review the technical considerations, fundamentals, and guideline-based indications for coronary artery calcium scoring, and the use of other non-invasive imaging modalities, such as extra-coronary calcification in cardiovascular risk prediction.Recent FindingsThe most robust evidence for the use of CAC scoring is in select individuals, 40 –75 years of age, at borderline to intermediate 10-year ASCVD risk. Recent US recommendations support the use of CAC scoring in varying clinical scenarios. First, in adults with very high CAC scores (CAC ≥ 1000), the use of high-intensity statin...

Effectiveness of lipid-lowering therapy on mortality and major adverse cardiovascular event outcomes in patients undergoing percutaneous coronary intervention: a network meta-analysis of randomised controlled trials
Conclusion EPA, especially icosapent ethyl, plus statins had a beneficial effect on reducing the risk of MACEs and mortality in post-PCI patients. Proprotein convertase subtilisin/kexin type-9 inhibitors plus statins were able to reduce the risk of MACEs, but the risk of mortality remained unclear. PROSPERO registration number CRD42018099600. Dyslipidemia in diabetes
Indian Heart J. 2023 Nov 11:S0019-4832(23)00175-X. doi: 10.1016/j.ihj.2023.11.002. Online ahead of print.ABSTRACTDiabetes mellitus is a metabolic disorder that often predisposes to cardiovascular diseases (CVD). CVD is an important cause of morbidity and mortality in diabetes. The typical diabetic dyslipidaemia is characterized by low HDL cholesterol, high triglycerides with mildly increased or even normal LDL. This attenuated rise in LDL is due to the more atherogenic small dense LDL particles. Genetic factors, obesity, lack of physical activity, alcohol abuse, poorly controlled glucose levels are some of the common risk ...

Clinical characteristics and treatment patterns in patients with atherosclerotic cardiovascular disease with hypercholesterolemia: a retrospective analysis of a large US real-world database cohort
CONCLUSIONS: This retrospective study highlighted limited LDL-C monitoring in patients with ASCVD, and unmet need in terms of suboptimal utilization of non-stain LLTs, limited adherence to LLTs, and inadequate lipid control after treatment (among those with LDL-C measurements during the follow-up period) need to be addressed to improve outcomes in this patient cohort.PMID:37941428 | DOI:10.1080/03007995.2023.2270901 (Source: Current Medical Research and Opinion)

Lipid-lowering in diabetes: An update
Atherosclerosis. 2023 Oct 6:117313. doi: 10.1016/j.atherosclerosis.2023.117313. Online ahead of print.ABSTRACTAtherosclerotic cardiovascular disease (ASCVD) is accelerated in people with diabetes. Dyslipidemia, hyperglycemia, oxidative stress, and inflammation play a role via a variety of mechanisms operative in the artery wall. In addition, some unique features predispose people with type 1 diabetes to accelerated atherosclerosis. Various organizations have created guidelines that provide advice regarding screening, risk assessment, and roadmaps for treatment to prevent ASCVD in diabetes. Management of dyslipidemia, espec...

Moderate-intensity statin plus ezetimibe vs high-intensity statin according to baseline LDL-C in the treatment of atherosclerotic cardiovascular disease: A post-hoc analysis of the RACING randomized trial
Whether the effect of a combination strategy rather than increasing doses of one drug to lower the low-density lipoprotein cholesterol (LDL-C) levels is consistent across the baseline LDL-C levels remains uncertain. (Source: Atherosclerosis)

Familial hypercholesterolemia and its manifestations: Practical considerations for general practitioners
Kardiol Pol. 2023 Nov 8. doi: 10.33963/v.kp.97845. Online ahead of print.ABSTRACTFamilial hypercholesterolemia (FH) is the most common genetic disorder of lipid metabolism, affecting almost 1 in 250 individuals worldwide. It is usually inherited via the autosomal dominant way and is characterized by aberrantly high total and low-density lipoprotein cholesterol (LDL-C) concentrations from early childhood, predisposing to increased risk of premature atherosclerotic cardiovascular disease (ASCVD), mostly coronary heart disease (CHD). Despite its high prevalence in the general population and the high ASCVD risk, FH is often un...
