So I don't do anesthesia much on really really big people....I know that is standard for many of you. But I have a question for those that do. I did a laprscopic case (weight loss surgery) yesterday. BMI 53. The patient was persistently hypoxia (84 to 89%) which I am 99% sure the shunting was from decreased FRC and atelecasis. Any of you guys doing the TENEX procedure? If so, are there courses to learn it? I had a couple weeks with the sports medicine guys and they were doing a lot of it. Guys, With the demise of medical bookstores and Amazon's crappy search results, I have experienced much buyer's remorse... Sigh, I miss the days of browsing bookstores. Use of ED ultrasound
I wanted to 'poll the audience' to see what others think about use of US in the ED. Of course it's helpful for the quick FAST or to place a central line, but I mean beyond that. I trained at a time when it was just on the up and up but they're now using teaching ER doctors to do the full gallbladder, DVT, and all pelvic scans on their own (amongst others). The Hospital where I work has recruited 2 orthopods starting mid feb 2017. The orthopods want regional blocks. The OR manager showed me a Hitachi ultrasound machine that was rusting in the OB department with ob us probe. OB Case: What would you do?
On call the other night and get a call from OB resident at 2am that there is a patient that was scheduled for a C-section the upcoming day for 7:30am. PAtient had come to OB office that afternoon due to feeling contractions around 4PM. In office patient was found to be 3cm and sent to hospital for observation, ultrasound and cerv exams. PT Aide responsibilities in my PTCAS App
Hey guys. I've been a PT Aide for about 1.5 years in a clinic which lets me do everything but stretch the patients. So, I do ultrasound, put electric stimulation on patients, hot packs/cold packs, and do actual mayofascial release (massage) even though I am not certified. This place is obviously a little sketchy. Opinions on Ultrasound Machines - Sonosite M-Turbo vs GE Logiq e
We're looking to buy another ultrasound machine here. I've used the M-Turbo but not the GE. Anyone have any comments comparing them? Intended use would be PNBs and vascular access. No need to ever support TEE at this location. M-Turbo would come with the HFL38 linear probe (6 - 13 MHz) and the enhanced needle visualization software. private practice vs. ultrasound
Hello all, Can any kind and patient soul explain in very basic terms what is and is not reimbursable using ultrasound? I find this very confusing. In training I used it a ton, but I have found some in private practice who completely avoid it not because they don't know how to use it, but because they have found that its use would lead to significantly less reimbursement. GE ultrasounds
Our hospital is refusing to get us the Sonosite we are requesting and instead offering as a substitute the GE Venue 50. I (and most of my group) am quite partial to Sonosite, mainly because that's all I've used and I am very happy with them. Anybody used this one? Give me the dirt. Pediatric abdominal pain: CT, MRI, US ???
Having recently attended a pediatric ER conference I learned about this protocol for r/o appy in peds. Ultrasound followed by MRI if results are equivocal instead of CT. I had never heard of this, but I can see this starting to gain traction in the community. I'm curious about people's practices for imaging kids for r/o appy. Useful non-EM electives for Emergency Medicine applicants?
What non-EM electives do you think would be useful to take 4th year (maybe ones that EM residency training doesn't give enough practice in)? I've already done a SICU elective and am scheduled for a radiology and an EKG one, but I might still have some room for a few more 2 and/or 4 week. I've heard people say anesthesiology, derm, ophthalmology, ortho/sports med, Peds EM, Tox, ID, ultrasound, etc. And wanted to see what current attendings, residents, or even 4th years think. Idea for Personal Statement a turn-off for adcomms?
Background: I did a crowdfunding campaign to raise money to purchase an ultrasound machine for a hospital I volunteered at in Tanzania. It took eighteen months, but it worked: $8K and a new ultrasound. I'm thinking of introducing this as the first part of my PS in a "walk-you-through-my-thinking" (first person) style to demonstrate an ability to use problem-solving skills to help others.
