How can you know if you like EM because you like the specialty or because you just get to do a lot as a med student?
M3 here. Been interested in EM for some time now. With VSAS around the corner, I need to seriously decide whether or not I should pursue EM. One thing I've been thinking about as of late is if my interest in the field right now has been a function of the fact that I have been able to do a lot during my shifts (I've done 15 shifts), such as H&Ps, present to attendings, updating patients on results, and helping with ultrasound or some minor procedure. Online preceptor for non-clinical ultrasounds?
A few of us independent solo or small group family docs in our area are working on adding Point of Care Ultrasound (POCUS) but are having a hard time hiring a local or online preceptor to review scans we've done so we can get enough precepted scans to satisfy our med mal carrier. We've already had courses, have the equipment (mostly Butterflys) and are piling up scans but have yet to find a preceptor to review them with us. Mobile/pocket ultrasounds
Anyone have any cool ultrasounds that are highly mobile and have a relatively decent picture? The Butterfly IQ US looks really cool at a decent price (especially considering it's 3 probes in one), but it requires an annual subscription. I'm not doing. Ultrasound charge
I have been informed by our billers that charging for ultrasound requires too much documentation to continue billing. I know that ultrasound billing has been threatened for awhile, but I still thought they were allowing it. Has there been changes? Have you ran into this issue and/or found a solution? Fraud
I was recently hired as a consultant to review claims for services provided by about 2 dozen DPMs. The fraud and the abuse are staggering. Most of those involved are young associates in private practices. The practice owners impressed these associates with how much money they can make if they bill as instructed. I have reviewed cases for an initial visit with unilateral heel pain. The patient ended up with BILATERAL xrays, ultrasound and fluoroscopy all for “comparison”. Patients... Stellate Ganglion Block (cervical sympathetic chain block) with ultrasound guidance; in-plane or out of plane?
Assuming one starts at C6 or C7, Seems to me using an out of plane approach makes a lot of sense, for the following reasons: 1. easier to avoid vessels 2. Abdominal pain in a 20 year old female, what could this case be?
20 year old female presents to the ER with abdominal pain. Afebrile and normal vitals. Pregnancy test negative. Labs normal. Ultrasound normal, no stones. CT (resident read) shows nonspecific inflammatory findings. Patient is sent home and her chlamydia test later returns positive. Attending CT read finds significant inflammatory findings in the mesentery and omentum, and she is told to return to the ER. Negative GU physical exam. Subclavian lines
I’m a surgery Intern going into IR. And I am coming here with a small grievance.... someone I plead with you please someone tell what is the obsession with surgeons and subclavian lines!?!?!?! Step 3 ccs/consults?
Just did a question on uworld where patient needs a endarterectomy. I consulted vascular surgery but they said no recommendations at this time, even after getting a carotid ultrasound. I was reading up on previous threads and not sure what the right move is. On the real test, am I supposed to proceed with the endarterectomy if the consultant doesn't give any recommendations, or are they always supposed to give a recommendation if you sequence everything correctly on the real thing?
