A Hot Topic
A 58-year-old man presented unresponsive following a seizure at home. His brother stated that he became progressively confused over the course of a few hours and then started shaking. EMS reports tonic-clonic seizures that resolved following administration of 5 mg of midazolam IM. The patient was unresponsive and hyperthermic on arrival. He was intubated for airway protection, covered with ice packs, and administered normal saline intravenously. His rectal temperature is 41.9˚C (107.4˚F), blood pressure is 94/45 mm Hg, heart rate is 160 beats/minute, and the respiratory rate is 16 breaths/minute with an oxygen saturatio...

Eight Month-Old Baby Dies Just Weeks After Hepatitis B Vaccination; His Innocent Father Is Still In Jail 20 Years Later
Conclusion After reading Dr. Al-Batati’s report in full and hearing Bryant’s own account of what happened on the night that Jordan died, I believe that Bryant Arroyo is an innocent man and that this case is in fact an elaborate cover-up by the medical profession, the police and US judicial system to hide the facts of what really happened to baby Jordan on the night that he died. I say this because the facts speak for themselves. Jordan was an extremely sick little boy, particularly in the months prior to his death. On many occasions, doctors neglected to diagnose that he was suffering from a life-threatening genetic il...

Heartland virus disease
Six new cases of Heartland virus disease have been identified in residents of Missouri and Tennessee. The cause of this disease appears to be a member of the Phlebovirus genus in the Bunyaviridae family that was first identified in 2009 and appears to be transmitted by the Lone Star tick (Amblyomma americanum, pictured). Heartland virus was first identified in two Missouri farmers who were hospitalized with fever, leukopenia (low numbers of white blood cells), and thrombocytopenia (low numbers of platelets). Both were males over 55 years old who reported being bitten by ticks in the week before disease onset. The novel vi...

Notes to myself – 2
Pentobarb coma – BIS should be 10-20 and SR (suppression ratio) should be 70-80 Consider lev albuterol Should give vaccines after coiling of spleen or before if possible No calcium channel blockers post MI definitely and post op in general Toradol inhibits spine healing Don’t do endoscopes with patients in supine position don’t ambulate patients with known dvt’s. wait 2-3 days until clots get stuck. dvt’s even with filter get heparin as much as possible for post phlebitic syndrome and to retard new clot formation diffuse alveolar hemorrhage – secondary to chemo, goodpasture’s, wege...

Dogs And Cows And Toxic Vials, That’s What Vaccines Are Made Of!
Conclusion It is apparent that our vaccinations include some very strange and unsavoury ingredients. The three single vaccines were certainly not the only vaccines I came across containing parts of animals, humans and insects. In fact, I found a huge list of them on a PDF which I have included as extra research. For some reason, many parents are perfectly happy to have their tiny babies vaccinated with everything from insect cells to pigs’ gelatin, without a moment’s hesitation, arguing that the vaccines are to protect their children and keep them strong and healthy. Others remain totally unaware of the vaccine...

NYC Mayor is Calling a Meeting to Force Vaccinate Under 5′s With Two Dangerous Vaccines
Conclusion Surely, unless the mayor of New York City is a qualified doctor or scientist, he should not be recommending the force-vaccination of children as young as six months, no matter how good his intentions are, especially since the two vaccinations he is recommending are possibly completely incompatible with one another and could possibly lead to multiple child deaths in New York City and across America. This is absolutely unforgivable, and in my opinion, he should be severely reprimanded for his actions. I suggest that the public of America join forces and either attend this meeting in person to voice an opinion or s...

Medical Mispronunciations and Misspelled Words: The Definitive List.
Hearing medical mispronunciations and seeing misspelled words are an under appreciated  joy of working in healthcare.  Physicians often forget just how alien the language of medicine is to people who don't live it everyday.  The best part about being a physician is not helping people recover from critical illness. The best part is not  about  listening and understanding with compassion and empathy.  Nope, the best part about being a physician is hearing patients and other healthcare providers butcher the language of medicine and experiencing great entertainment in the process.   Doctors c...

Worrier In Chief
Saul couldn't have been a nicer guy. The story behind his arrival to the nursing home was long and sorted. But now we had a black gangrenous foot to deal with. The culprit, not the toe ulcer that brought him in to the hospital in the first place, but small thrombosed blood vessels from heparin induced thrombosis and thrombocytopenia, a reaction to a medication given as a precaution. The vascular surgeon was equivocal, amputation versus watchful waiting. Toes can autonecrose (self amputate), but when the black tide of dead tissue spread towards the ankle our options became limited. As the white count began to rise, I had t...

Cases: Anti-epileptic Medicines for Pain Management
Discussion:  Tri-cyclic antidepressants (TCAs), serotonin-norepinephrine reuptake inhibitors (SNRIs) and anti-epileptic drugs (AEDs) are the mainstays of adjuvant therapy for neuropathic pain.  This Case of the Month will focus on oral anti-epileptic neuropathic pain analgesics. Due to lack of head-to-head data, evidence is presented as numbers needed to treat (NNT) and numbers needed to harm (NNH). For instance, an NNT of 5 for 50% pain reduction means for every 5 patients treated with a drug, only 1 of them would achieve a 50% reduction in pain. Gabapentin (Neurontin) and pregabalin (Lyrica) are considered firs...

RESPECT: More thoughts for interns and residents courtesy of Dr. Darilyn Moyer
Darilyn Moyer is Vice-Chair for Education in the Department of Medicine at Temple University.  She also is the Chair-Elect of the ACP Board of Governors and a good friend.  She sent this wonderful piece on respect for house officers: The Power of RESPECT (one of my favorite songs  of all time) 1. Respect the Power of Words Perform bloodless surgery with your brain and practice a thorough history and physical. Despite the explosion of medical technology over the past several decades, most diagnoses can still be arrived at with a thorough history and physical.  The best doctors use technology to AUGMENT not SUBSTITUTE ...
