Think About Klippel –Feil as Causing Neurogenic Thoracic Outlet Syndrome!
A 49-year-old woman presented with bilateral symptomatic neurological and arterial thoracic outlet syndrome. Computed tomography angiography revealed bilateral cervical ribs (black arrows), synostosis with the first right rib (B,*), and a post-stenotic dilatation of the right subclavian artery (A, white arrow). The presence of a supernumerary hemi-vertebra at the cervicothoracic hinge and fused C6 – C7 cervical vertebrae (star) supported the diagnosis of Klippel–Feil syndrome. Transaxillary resection of the cervical and first ribs and scalene muscles was first performed on the right. (Source: European Journal of Vascul...

Migraine Headaches in Patients with Upper Extremity Compressive Neuropathy
Conclusions: Patients who undergo median and multiple nerve decompression are more likely to experience migraine headache. It is important to recognize this overlap and provide comprehensive patient screening for both conditions. This shared predisposition and better understanding of a common disease mechanism and genetics may provide greater insight into the pathogenesis and therefore treatment of these clinical problems. CLINICAL QUESTION/LEVEL OF EVIDENCE: Risk, III. (Source: Plastic and Reconstructive Surgery)

Factors Associated with 30-Day Adverse Events After Brachial Plexus Neurolysis
CONCLUSIONS: Episode-of-care postoperative complication and reoperation after brachial plexus neurolysis are associated with operative time, which may be partially related to surgical complexity, additional procedures, and/or surgeon experience. Rib resection procedures are associated with a higher risk of readmission.CLINICAL RELEVANCE: Our findings may be applied to preoperative risk stratification and patient counseling. Our research questions the role of bony rib resection in the routine treatment of neurogenic thoracic outlet syndrome when soft tissue release alone may be successful. Future research is necessary to de...

Current Clinical Concepts: Rehabilitation of Thoracic Outlet Syndrome
J Athl Train. 2022 Nov 17. doi: 10.4085/1062-6050-138-22. Online ahead of print.ABSTRACTThoracic outlet syndrome (TOS) involves an inconsistent set of symptoms that provides a challenge for medical providers to treat. Three categories of TOS are neurogenic, arterial, and venous. A combination of symptoms can occur and are most seen in overhead athletes. Symptoms may include pain, numbness, of an athlete to perform at their prior level of sport is impacted. Due to the variance in presentation and poor objective testing, there is not a well-defined examination process. It is suggested to perform an individualized assessment ...
