Faculty Perspectives on Responding to Microaggressions Targeting Clerkship Students
On this episode of the Academic Medicine Podcast, Meghan O’Brien, MD, MBE, and Research in Medical Education (RIME) Committee members Tasha Wyatt, PhD, and Javeed Sukhera, MD, PhD, join host Toni Gallo to discuss new research into faculty perspectives on responding to microaggressions targeting medical students in the clinical learning environment. They explore several tensions that affected how faculty responded to the microaggressions in the study scenarios as well as some of the strategies the faculty used to respond effectively. This episode is now available through Apple Podcasts, Spotify, and anywhere else p...

Sunday Sermonette: Elihu yammers on
I will address the question of the sustainability of Medicare and Social Security anon. I will say that while I don ' t know that there ' s a single, unified Democratic plan there are certainly some viable proposals. Unfortunately, right now I ' m stuck with posting more endless gibberish.  Fortunately, today ' s post concludes our encounter with the logorrheic Elihu, after which the story will begin to move on.36 And Eli ′hu continued, and said:2 “Bear with me a little, and I will show you,    for I have yet something to say on God ’s behalf.3 I will fetch my knowledge from af...

Lifestyle Changes
I looked at my last blog post. It was from June 28, 2021. I had just met my new primary care physician. Since then we had made slow changes to my health. I tried to eat better and exercise more. It has been a journey of trying to implement what I preach. The progress was slow. Last year, to help my physical activity endeavors (and because I have a huge interest in martial arts), I decided to start training at a local Karate school. The school I attend teaches Enshin Karate a style that traces it’s roots back through Ashihara and Kyokushin. Shortly after that time I noticed my Apple Watch was notifying me abou...

Understanding RNA-Modifying Enzymes: Q & A With Jeffrey Mugridge
Credit: Courtesy of Jeffrey Mugridge. “One of the best aspects of research is the excitement of discovery, being the first person in the world to know a small detail about the system you’re studying,” says Jeffrey Mugridge, Ph.D., an assistant professor of chemistry and biochemistry at the University of Delaware in Newark. We talked with Dr. Mugridge about how a pet store job sparked his early interest in science, why he decided to change his career trajectory after graduate school, and what he believes is key to being a successful researcher. Q: How did you first become interested in science? A: ...

Wednesday Bible Study: Yadda yadda yadda
S í, estoy en Puerto Rico. As this endless vapid discourse meanders on, I will just say that although the Book of Job is as far as we know the earliest work in the Judeo-Christian tradition to deal with the theodicy problem, it ultimately fails to do so. The three comforters and Elihu all basically say the same thing, in the end: Job must have done something to deserve this, either because all humans are sinners or he transgressed in some particular way. Job says he is righteous and that God appears to him arbitrary and inscrutable. Sometimes the other four characters more or less agree with that, but still, th...

Faculty and Student Perceptions of Unauthorized Collaborations
In this study, it was clear students do recognize the need for individual accountability and that their individual competence will be assessed, but they also recognize that they are encouraged to work with each other and that throughout their careers they will be working with colleagues in the clinical settings and for the rest of their lives. That gets at some of the tension, and we create some of that tension in the curriculum because we foster students working together in small groups and we embrace them supporting each other and helping each other learn. Then that comes up against the individual demonstration of compet...

Learning health equity from activists: a paradigm shift for physicians
The practice of evidence-based medicine and lifelong learning are core tenets of medicine. Through courses, conferences, and lectures, physicians have regularly turned to their colleagues for continuing education. For decades, this reliance on physician-to-physician teaching has been a fitting choice for the subjects at hand, which usually related to clinical practice. In recent years, there Read more… Learning health equity from activists: a paradigm shift for physicians originally appeared in KevinMD.com. (Source: Kevin, M.D. - Medical Weblog)

Does this T wave pattern mean anything?
Written by Michael Doyle DO and Timothy Palmieri MD. Edited by Bracey, Meyers, Grauer, and SmithA 50-something-year-old female with a history of an unknown personality disorder and alcohol use disorder arrived via EMS following cardiac arrest with return of spontaneous circulation. The described rhythm was an irregular, wide complex rhythm. By EMS report, open pill bottles were found nearby at the scene, including quetiapine, fluoxetine, hydroxyzine, and gabapentin. She was successfully revived after several rounds of ACLS including defibrillation and amiodarone. On arrival to the ED the patient was intubated with normal vi...

Sunday Sermonette: Yet another blowhard
It seems that four endless yammerers isn ' t enough. Now we get a fifth. Elihu runs his mouth for the next six chapters and says approximately nothing. He spends the first chapter just blathering about how he is about to speak. Then he says that Job is wrong to claim he hasn ' t done anything to deserve his fate, but the other three Bozos are losers because they haven ' t been able to prove that he ' s wrong. But then Elihu spends the next four chapters equally failing to prove anything and just spewing more endless dreck about the greatness of God. This book could easily be one twentieth as long, probably a lot less, and ...

Aspects of Social Determinants of Health: Specific Types of Help
This is the final article in a series about social determinants of health (SDoH). Previous articles covered the definition, collection, sharing, and use of SDoH. I’ll finish with a look at some specific areas of intervention. We’ve seen the importance of personal interventions, through interviews and evaluating SDoH, to solve problems. In rural areas, where it might be daunting either to travel to a doctor or to schedule an online visit with poor Internet access, home visits fill the gap. Conduit Health Partners is one organization that does so. A patient can speak to a nurse immediately—as opposed to calling...

How Healthcare Communication Platforms Can Harness Generative AI in a HIPAA-Compliant Way
The following is a guest article by Nate MacLeitch, Founder and CEO at QuickBlox The demand for AI-powered Communications Platform as a Service (CPaaS) in healthcare is evident: 75% of executives believe that generative AI has reached a stage where it is poised to reshape the healthcare industry. When the sector is set to face shortages of between 37,800 and 124,000 physicians by 2034, generative AI could alleviate administrative work, such as summarizing patient calls and managing electronic health records (EHRs), to allow medical staff to focus on improving patient care. Yet, privacy laws and a fragmented market have kep...

Aspects of Social Determinants of Health: Acting on SDoH
Previous articles in this series introduced the importance of social determinants of health (SDoH) and explained how it can be captured and classified. In this article, I’ll show how many hospitals, payers, and other institutions are acting on that data. Of course, the staff of a clinic or hospital don’t go around looking for apartments the way they actively look for a long-term care facility. Instead, their case managers partner with community institutions to help patients. But the clinic or hospital is a crucial linchpin in connecting the patient to this help. According to Dr. Victor Lee, vice president of cl...
