Caremark quarterly clawbackd
Being a new manager, I have been exposed to the quarterly reports from the PBMs and I just got the caremark one in the other day. Please help me understand and suggest how to go about these:- 1. Gap therapy for statins- how can we let the plans know that doctors don’t wanna just blindly prescribe statin? And if they are discontinued, are they getting accounted for in the metrics? 2. Would you ever consider prescribing statins?
I spend half my week in a community clinic working with a lot of folks with schizophrenia or bipolar 1 so have fair number on SGAs either PO or LAI. I’m constantly getting lipid panels back that are off the chain, refer to primary care and seems less than 20% of patients ever actually go. Currently I would never consider prescribing statins because I’m not up on guidelines, but has gotten me starting to wonder. I can check any labs I want and presumably it’s not rocket science to follow... Would you ever consider prescribing statins? (Source: Student Doctor Network)

Nontrad hopefully unrealistic fear of mortality
I started medical school late, I will graduate at 33, and be done with residency at 37. I had a cholesterol check up and my cholesterol pretty much sucked. 2 years ago I was borderline high, I changed my diet and began exercising again. Just had a check up and my cholesterol is worse. I am begging them to put me on a statin. I plugged in my numbers into the framingham 30 year estimate and I have a 8% chance of "Hard CVD" - death or a major event, and a 14% chance of complications. HIV medication drug interactions
I am studying for my consult psych recert boards. Various sources indicate that ritonavir and other HIV antivirals tend to lower methadone levels. According to the UCSF site, ritonavir may induce 3A4, thus lowering methadone levels: Interactions with Methadone. However, other sources, including wikipedia, states the ritonavir inhibits 3A4 - thus increasing the levels of drugs such as statins. the package insert... Public service announcement: CPJE is incredibly difficult
I just took the CPJE. It was awful and I'm sure I failed. There were so many questions that were obviously written based on guidelines from a decade ago. I never learned these old guidelines, I learned the current ones! There was a question about the proper statin dose for someone based on their cholesterol... We were told in school NEVER to do this, and to use ASCVD risk (per the newer guidelines). There were a ton of questions about niche disease states like Myasthenia Gravis. There... Modern Medicine does little to extend life
Elsevier: Article Locator Great article discussing how life is really only extended through smoking cessation, physical activity, stress reduction, weight control and good sleep. My dad had blood work recently which showed he had high cholesterol. His family physician advised him to go on a statin.
