What next for the polyclinic? New models of primary health care are required in many former Soviet Union countries
CONCLUSION: The comprehensiveness of care in a big setting and potential economies of scale, which are major strengths of the polyclinic model, should be retained in the provision of specialty care rather than primary care. Internationally, there are lessons about the risks associated with models based on narrow specialization in caring for patients who increasingly have multiple conditions.PMID:35927680 | DOI:10.1186/s12875-022-01812-w (Source: Primary Care)

"I Can't Take This Shitty Quarantine Anymore": Sexual Behavior and PrEP Use Among Young Men Who Have Sex with Men and Transgender Women in Brazil During the COVID-19 Pandemic
This study analyzes how the COVID-19 pandemic affected sexual behavior and use of HIV prevention methods among young transgender women (YTGW) and young gay, bisexual and other men who have sex with men (YGBMSM) participating in an HIV pre-exposure prophylaxis (PrEP) demonstration study in Brazil. Online interviews with 39 participants aged 15-22 years old were conducted between September and November 2020 and analyzed based on social constructionism and human rights-based approaches to health. Management, Functional Outcomes, and Quality of Life After Development of Pelvic Sepsis in Patients Undergoing Re-Do Ileal Pouch Anal Anastomosis
CONCLUSIONS: Pelvic sepsis is common after re-do IPAA, and management varies according to the location and size of the abscess/sinus. If detected early, our management strategy was associated with high pouch salvage rates. See Video Abstract at http://links.lww.com/DCR/B823. Referring Provider Opinions of Pediatric Cardiology Evaluations Performed by Nurse Practitioners
AbstractIn the setting of physician shortages, nurse practitioner (NP) roles have evolved, with increasing independence across most healthcare settings. We sought to characterize referring clinician perceptions of NP-performed outpatient pediatric cardiology consultations. We electronically distributed to pediatric and family medicine physicians and NPs in Arkansas our 11-item survey assessing the acceptability of pediatric cardiology consultations being completed by an NP under varying circumstances. Circumstances included seven common referral indications, and the scale offered five answer choices ranging from “definit...

Reducing New Ileostomy Readmissions in a Rural Health Care Setting: A Quality Improvement Initiative
CONCLUSION: Implementation of initiatives targeting enhanced team continuity, the standardization of rehydration therapies, and improved patient education decreased readmission rates in patients with new ileostomies. Rural centers, where outpatient resources are not as readily available or accessible, stand to benefit the most from these types of targeted interventions to decrease readmission rates. See Video Abstract at http://links.lww.com/DCR/B771. The norms and practices of eating well: In conflict with contemporary food discourses in Japan
Appetite. 2022 May 20:106086. doi: 10.1016/j.appet.2022.106086. Online ahead of print.ABSTRACTIn contemporary society, numerous food discourses are being constructed and promoted regarding how and what people should eat. However, such expositions are often too nutrition-centred and contradict each other, leaving contemporary eaters at a loss rather than rescuing them from their gastro-anomic situations. It is therefore necessary to seek an understanding of what 'eating well' means to contemporary people and to navigate current food discourses based on such a social consensus. Using a combined method of ethics (capability a...
