What would be most beneficial for me to do this summer? Research or clinical experience?
So I'm a junior in college, my major is Behavior and Health. I had been planning to be an Occupational Therapist for a WHILE but last month I just realized I was picking that career because I thought I wasn't "smart enough" to go into research/PhD work like I wanted to back in high school. treatment resistant bipolar/schizophrenia?
At my job I've inherited some of the more complex cases after someone in my hospital system left. It is a population of veterans who get intensive case management and a number of them are diagnosed with treatment resistant schizophrenia/bipolar disorder. Classically, I was taught that if someone fails two antipsychotic trials, it is not unreasonable to consider clozapine and there is not much evidence for multiple antipsychotics. Is this an unusual reason to go into psychiatry?
I am pursuing psychiatry and when people ask my reason, I tell them: good hours, low stress, low burnout, controllable lifestyle While all of these are true, there's more to it. *This might be TMI * I was abused as a child, both sexually and physically, over the course of a few years. Behavioral neurology and general psychiatry?
I know behavioral neurologists typically treat or manage psychiatric issues associated with organic, neurological disorders such as psychosis from alzheimers for instance. But can a behavioral neurologist manage psychiatric symptoms of disorders of unknown etiology? Or those without a visible neurological basis such as PTSD or schizophrenia? Behavioral neurology and mental illness?
Can a behavioral neurologist treat certain mental illness such as schizophrenia? Or are they only allowed treat behavioral/ cognitive symptoms that result from a known lesion or pathology in the nervous system? With that being said, couldn't schizophrenia technically be considered a "functional neurological disease" since professionals and scientists know there are neurobiological components but haven't pinpointed any structural deficits (as of yet)? What is it about schizophrenia that makes it practically an exclusively psychiatric disease as opposed to having some neurological cross over? Hyponatremia and schizophrenia (or any other psychiatric condition)
Common occurrence that I see at my hospital weekly: psych pt with hyponatremia presents to ED, admitted for med/psych eval and more often than not psychogenic polydipsia is Dx. NS until Na is resolved and get them out the door. But there is one case that I haven't been able to stop thinking about. Middle aged man w/ PPHx schizophrenia and depression, PMHx HTN comes to ED from ALF, routine labs show Na 113.1 and Sosm 230.5. Patient was AOx3, vitals WNL. No labs for Uosm ordered and yet this...
