Hello, I was recently accepted to a medical school this last month, however, over winter break I had a mental episode and was convinced I was getting schizophrenia for the first ~6 weeks. dexmedetomidine (POPrecedex) for agitation, and maybe delirium, and maybe dementia with behavioral disturbance?

Does this job exist -working mostly with patients who have schizophrenia, schizoaffective, bipolar 1, and/or OCD. Options for clozapine-resistant schizophrenia?
I have a patient on the long term ward who transferred from a local short term ward, presented with persisting persecutory delusions and command hallucinations with accompanying violence risk, dx was schizophrenia. SIADH and Risperidone
I have an outpatient over 60 with schizophrenia who has had suspected SIADH associated with Risperidone. Polydipsia is not present. Hyponatremia improved with reduction in Risperidone from 5mg daily to 2mg daily, but unfortunately paranoid delusions and auditory hallucinations that were controlled on 5mg returned. Does anyone have experience with SIADH and atypical antipsychotics? A review of the literature has not been helpful so far in differentiating one atypical antipsychotic with another. I had an interesting moment the other day while working with an acute patient at my hospital. This particular patient has a longitudinal history of debilitating schizophrenia even whilst medicated, and most, if not all of his family has left him high and dry. Was working on our consult service and had an interesting case of substance-induced vs. first-break schizophrenia as the primary diagnosis, but during one of the interviews the patient happened to mention having the belief that his penis had been gradually retracting into his body for the past year. Further questioning seemed to be pretty spot-on with Koro as a diagnosis, but my attendings were mixed on whether this was an appropriate diagnosis because the patient was white and the culture...
