False Sexual Harassment Complaint--Male Psychiatrists Beware!
OK, I am an older male NP (from a real established, "brick and mortar" midwestern nursing school, not a on-line type), and here is my story. I work in a community mental health clinic. It is me, one other NP, and a great psychiatrist-we manage the medications of over 2000 hardcore psych patients. Probably 99.5% are Medicaid, and 70% are on SSI, or SSDI. Lots of schizophrenia, schizoaffective d/o, bipolar d/o, and BORDERLINES! I have never had any issues in over 30 years of psych... False Sexual Harassment Complaint--Male Psychiatrists Beware! Child Psychiatry Grand Round suggestions?
Hey everyone, Been browsing for a while and finally created an account. I'm finishing up my Child Psych fellowship and I was wondering if anyone had any good suggestions on a good topic to do a Grand Rounds on. I have to give a talk in a couple months on a topic that would be seen amongst fellows and residents. I was thinking about Childhood Onset Schizophrenia and Autism but want something maybe not as broad. Any suggestions from SDN folks? Thanks! (Source: Student Doctor Network)

Capacity for ECT
I have elderly pt on CL service, he has catatonic schizophrenia and does not have capacity to make decisions, his daughter is his next of kin and also his HPOA, his daughter refuses any type of psychotropic medication but wants to do ECT, can we do ECT with her as surrogate decision maker or do we need to probate him before ECT? Thx for insights (Source: Student Doctor Network)

Sudden onset psychosis lasting months
Pt is a 20-something male. A few months ago, "something happened" at work that he can't expound upon. Within one day, he slumped into depression with minimal talking. He became paranoid, responds to internal stimuli, made references to "thought insertion" regarding aliens and demonstrates disorganized speech and behavior. MRI, syphillus and A/I workup wnl. This has been going on for months. What could this be? Can schizophrenia come on this suddenly? (Source: Student Doctor Network)

Medicare keeps denying labs
Hi, I have been taking medicare patients for a while. lately after a local community center went bankrupt, i decided to take a few patients. One current issue I keep having is that medicare keeps denying coverage for labs like cbc, chem panel, lipids, hemoglobin A1c. The labs (quest/labcorp) don't know what it is either other than we have the wrong codes. I am using the standard codes for bipolar/schizophrenia. F31.x, F20.x etc. Anyone else having this issues/have experience with this? (Source: Student Doctor Network)

The Six Personal Statement Types in Psychiatry *rehashing classic post*
The Six Personal Statements in Psychiatry Previously posted in 2015, this seemed to have disappeared but reposting here due to popular demand. It was not well received at the time by neurotic med students however. These examples are much better written than the actual detritus we read. 1. My Mentally Ill Relative "Schizophrenia is one the most frightening and devastating diseases known to man. For most medical students, the experience of this fear and devastation is... Would you ever consider prescribing statins?
I spend half my week in a community clinic working with a lot of folks with schizophrenia or bipolar 1 so have fair number on SGAs either PO or LAI. I’m constantly getting lipid panels back that are off the chain, refer to primary care and seems less than 20% of patients ever actually go. Currently I would never consider prescribing statins because I’m not up on guidelines, but has gotten me starting to wonder. I can check any labs I want and presumably it’s not rocket science to follow... Would you ever consider prescribing statins? (Source: Student Doctor Network)

New article: Ketogenic diet put schizophrenia into remission in two patients
Chronic Schizophrenia Put Into Remission Without Medication New research suggests ketogenic diet may play a role in treating schizophrenia. www.psychologytoday.com One question I had was about this part: "Interestingly, the effects of this diet on the brain have been studied for decades because neurologists have been trying to figure out how it works in epilepsy. This diet is known to produce ketones which are used as a... New article: Ketogenic diet put schizophrenia into remission in two patients (Source: Student Doctor Network)

Conveying research interests in a statement of purpose
Hi there! I was wondering if you all might be able to offer some advice about conveying research interests in a statement of purpose in prep for applying to Clinical PhD programs? I know the population I'd like to specialize in (schizophrenia spectrum disorders) and a broad idea of the topics I'm particularly interested in studying (social cognition, social motivation, amelioration of cognitive deficits, promotion of metacognition, recovery-oriented therapy methods for working with people... Differential diagnosis/med recommendations
Patient is a 60 yo Black female diagnosis of Schizophrenia. Client had first psychotic episode at the age of 30 after her abusive husband left her. Due to not having childcare, she had to quit her job and lost her home, which caused her children to be removed from her care. At this point, she entered into a depressive episode and experienced psychosis (she thought she was pregnant despite doctors consistently telling her she was not). Fast forward 30 years, client is stable, has been on... Differential diagnosis/med recommendations (Source: Student Doctor Network)

Agitated patient on vent
So I was wondering. We had this patient who was found unresponsive foaming in the mouth and was intubated for airway protection. She has schizophrenia and is on a bunch of psych meds. We ve been unable to wean her off sedation because she gets extremely agitated. My question is if it was only for airway protection why not just take her off sedation completely take her off the vent and then control her agitation. why does she still need to be on the vent (Source: Student Doctor Network)

Letter of Recommendation advice please??
Okay so last semester I studied abroad and interned at mental health outpatient facility for individuals with schizophrenia, bipolar disorder, and unspecified personality disorders. It was a really great experience and my supervisor really really liked me. I asked him to write me a letter of recommendation for an REU this summer and he agreed. I emailed him about it on like December 25th, just sending him my resume, my research interests, transcript, and link to submit the letter; however,... Letter of Recommendation advice please??
