Cervical carotid occlusion in acute ischemic stroke: Should we give tPA?
CONCLUSION: Emergent revascularization with acute cICA stenting carries advantages, but its safety is precluded by tPA administration. We suggest a trial which randomizes patients with cICA occlusions to receiving either tPA or dual antiplatelet therapy before surgical intervention, aiming to ultimately improved outcomes in these patients.PMID:35509556 | PMC:PMC9063023 | DOI:10.25259/SNI_176_2022

Safety and Efficacy of Tirofiban During Intravenous Thrombolysis Bridging to Mechanical Thrombectomy for Acute Ischemic Stroke Patients: A Meta-Analysis
ConclusionThe use of tirofiban during IVT bridging mechanical thrombectomy for AIS does not increase the risk of sICH and ICH in patients and reduces the risk of postoperative re-occlusion and mortality in patients within 3 months. However, this result needs to be further confirmed by additional large-sample, multicenter, prospective randomized controlled trials.Systematic Review Registrationhttp://www.crd.york.ac.uk/PROSPERO/, identifier: CRD42022297441.

Acute ischemic stroke in children & #8211; should we thrombolyze ?
KP Vinayan, Vivek Nambiar, Vaishakh AnandAnnals of Indian Academy of Neurology 2022 25(1):21-25 Ischemic stroke is a major cause of acute neurological symptoms in children with significant long-term neurological sequelae. Unlike in the adult population, the clinical presentation of strokes in children may not be stereotyped. Hence, many other differential diagnostic possibilities might have to be considered in the emergency setting. Due to this heterogeneous presentation and the resultant clinical dilemma in the early detection, acute thrombolysis even now remains as a very rarely tried therapeutic option in children. Man...

Efficacy and safety of rt-PA intravenous thrombolysis in patients with wake-up stroke: A meta-analysis
Background : Recombinant tissue plasminogen activator (rt-PA) is one of the most effective therapies for patients with acute ischemic stroke. However, wake-up stroke (WUS) is typically excluded from intravenous thrombolytic therapy because of the unclear time of symptom onset. Therefore, we aimed to assess the efficacy and safety of rt-PA intravenous thrombolysis in patients with WUS by meta-analysis. Methods : We completed a systematic literature search of PubMed, Embase, the Cochrane Library, and SinoMed and included relevant studies of WUS patients covering rt-PA thrombolysis and nonthrombolysis (published fr...

Effect of pre-hospital early intervention combined with an in-hospital emergency model in the emergency care of patients with acute stroke
CONCLUSION: Pre-hospital early intervention combined with in-hospital emergency model in the first aid of patients with acute stroke can effectively improve the success rate of rescue, shorten the duration of rescue and length of hospital stay, reduce the incidence of complications, increase nursing satisfaction, and improve limb motor function and ADL.PMID:35173885 | PMC:PMC8829593
