Cost Effectiveness of Novel Oral Anticoagulants for Stroke Prevention in Atrial Fibrillation Depending on the Quality of Warfarin Anticoagulation Control
Conclusion The cost effectiveness of NOACs for stroke prevention in patients with nonvalvular atrial fibrillation who are at increased risk for stroke is highly sensitive to warfarin anticoagulation control. NOACs are more likely to be cost-effective options in settings with poor warfarin management than in settings with better anticoagulation control, where they may not represent good value for money.

‘It was like he was in the room with us’: patients’ and carers’ perspectives of telemedicine in acute stroke
ConclusionAlthough telemedicine was well accepted by many participants, its use added an additional layer of complexity to the acute stroke consultation. The ‘remote’ nature of the consultation posed challenges for some patients. These issues may be ameliorated by clear information for patients and carers, staff interpersonal skills, and teamworking.

Who does it first? The uptake of medical innovations in the performance of thrombolysis on ischemic stroke patients in Germany: a study based on hospital quality data
Conclusions: Because ischemic stroke patients benefit from systemic thrombolysis, it is necessary to examine possible barriers at the organizational level that hinder the implementation. Our data shows that, organizational factors have an influence on the implementation of thrombolysis. However, the recent guideline update resulted in a TR rise that occurred at all hospitals, regardless of the measured structural conditions, as our analysis could not identify any structural factors that might have influenced the TR after the guideline update.

Identifying reprioritization response shift in a stroke caregiver population: a comparison of missing data methods
Conclusions Test procedures based on relative importance measures are sensitive to the type and amount of missing data and imputation method. Relative importance test procedures based on mean imputation and multiple imputation are recommended for detecting RS in incomplete data.

Validity of EQ-5D-5L in stroke
Conclusions Results support the validity of the EQ-5D-5L descriptive system as a generic health outcome measure in patients with acute stroke, demonstrating some psychometric advantages in comparison with EQ-5D-3L.

Acute ischaemic stroke patients - direct admission to a specialist centre or initial treatment in a local hospital? A systematic review
Conclusions For ischaemic stroke patients, the location of initial thrombolysis treatment does not affect outcomes. However, if thrombolysis is only available at a specialist centre, outcomes are considerably better for those patients admitted directly. However, these conclusions are based on poor quality data with small sample populations, significant heterogeneity and subject to confounding.

Short- and longer-term health-care resource utilization and costs associated with acute ischemic stroke
Conclusion: In addition to the substantial costs of the initial hospitalization of an AIS, these costs double within the year following this event. Given the high cost associated with AIS, new interventions reducing either the acute or longer-term burden of AIS are needed. Keywords: acute ischemic stroke, health-care resource utilization, health-care costs, readmissions

Quality of life in aphasic patients 1 year after a first stroke
Conclusion Results and their practical relevance in treatment were discussed. Predicting as soon as possible which factors would be related to late QoL in stroke patients with aphasia is of major importance.

Does the 'diffusion of innovations model enrich understanding of research use? Case studies of the implementation of thrombolysis services for stroke
Conclusions While recognising the usefulness of the model of diffusion of innovations in mapping the processes by which diffusion occurs, the use of methods that lend themselves to in-depth analysis, such as ethnography and the application of relevant bodies of social theory, are needed.

Cost-effectiveness analysis of mechanical thrombectomy with stent retriever in the treatment of acute ischemic stroke in Italy.
CONCLUSIONS: MT plus IV t-PA for AIS patients with LVO is cost-effective from year 1 through year 3 and cost-saving from year 4 onward in the Italian context achieving better results, both in terms of efficacy and in terms of resource consumption. PMID: 29882711 [PubMed - as supplied by publisher]

A new era in stroke care gives larger treatment window and better outcomes
According to the American Heart/American Stroke Association, stroke affects nearly 800,000 people in the United States each year. It is the fifth leading cause of death, and the leading cause of long-term disability.  It’s also the leading cause of preventable disability. In China it is the leading cause of death surpassing both cancer and heart disease. Stroke occurs when a blood vessel carrying oxygen and nutrients to the brain is either blocked by a blood clot or plaque (ischemic stroke), or…

Long-term health benefits of stroke prevention with apixaban versus vitamin K antagonist warfarin in patients with non-valvular atrial fibrillation in Germany: a population-based modelling study.
CONCLUSION: This study demonstrated that using apixaban instead of VKA for stroke prevention can lead to considerable reduction in cardiovascular events. PMID: 30295561 [PubMed - as supplied by publisher]

Stroke Ready: a multi-level program that combines implementation science and community-based participatory research approaches to increase acute stroke treatment: protocol for a stepped wedge trial
Post-stroke disability is common, costly, and projected to increase. Acute stroke treatments can substantially reduce post-stroke disability, but few patients take advantage of these cost-effective treatments....

Number needed to treat based on real-world evidence for non-vitamin K antagonist oral anticoagulants versus vitamin K antagonist oral anticoagulants in stroke prevention in patients with non-valvular atrial fibrillation.
CONCLUSIONS: The NNT calculation, when approached and interpreted properly, is a practical measure of the effectiveness of a treatment. The calculation based on HRs showed that NOACs are safe and effective alternatives to VKAs in real life. PMID: 30969801 [PubMed - as supplied by publisher]

Ischemic stroke: clinical pathway impact
International Journal of Health Care Quality Assurance,Volume 32, Issue 3, Page 588-598, April 2019. Purpose A clinical pathway for patients with acute ischemic stroke was implemented in 2014 by one Italian teaching hospital multidisciplinary team. The purpose of this paper is to determine whether this clinical pathway had a positive effect on patient management by comparing performance data. Design/methodology/approach Volume, process and outcome indicators were analyzed in a pre-post retrospective observational study. Patients ’ (admitted in 2013 and 2015) medical records with International Classification of Diseases, ...
