Interorganizational Relationships in the Heart and Stroke Foundation's Spark Together for Healthy Kids™: Insights From Using Network Analysis
The Heart and Stroke Foundation’s Spark Together for Healthy Kids™ (Spark) is a multiyear initiative in Ontario, Canada, that takes a population approach to obesity prevention. It focuses on creating healthy environments by improving access to healthy foods and physical activity, with an emphasis on strengthening the advocacy capacity of organizations and citizens. Individualised home‐based rehabilitation after stroke in eastern Finland – the client's perspective
This study focused on clients' experiences of a 3‐month individualised, home‐based rehabilitation programme supervised by a multidisciplinary team. The data were collected in 2009–2010, and it was based on interviews with 14 clients (48–83 years of age) conducted approximately 7 months after stroke. In the thematic analysis, five main topics describing the goals and functions of the home‐based rehabilitation were identified as follows: (i) learning strategies for solving problems in daily activities at home and in the community; (ii) receiving exercise coaching; (iii) exploring community services and facilities...

Stroke-Related Sarcopenia: Specific Characteristics
Sarcopenia is characterized by muscle wasting and is primarily a disease of the elderly. A stroke-specific sarcopenia has been described recently. Stroke-related sarcopenia has a number of features that distinguish it from the age-related sarcopenia. The disability from stroke depends on the brain lesion leading to impairment of the efferent neuronal pathways. However, the alterations of structural and functional muscle capacity are secondary and depend rather on complex pathophysiological reactions including imbalanced efferent neurovegetative control, systemic and local metabolic imbalance, feeding difficulties, and inflammation.

The Educational Needs of Clinicians Regarding Anticoagulation Therapy for Prevention of Thromboembolism and Stroke in Patients With Atrial Fibrillation
The objective is to identify practice patterns and attitudes of and barriers faced by US physicians assessing thromboembolism/stroke risk and managing anticoagulation in atrial fibrillation (AF) to determine educational needs. Case-based surveys were used to assess practice patterns, guideline use, barriers, and attitudes; 51 cardiologists and 50 primary care physicians (PCPs) were surveyed. Most cardiologists use validated risk scoring systems to assess thromboembolism/stroke risk, and more than half of PCPs use clinical experience. Assessment of bleeding risk varied; more than half of respondents rely on clinical judgmen...

A Decision Analysis of Percutaneous Left Atrial Appendage Occlusion Relative to Novel and Traditional Oral Anticoagulation for Stroke Prevention in Patients with New-Onset Atrial Fibrillation
Conclusions. Apixaban is the most cost-effective therapy for stroke prevention in patients with new-onset NVAF without contraindications to OAC. Uncertainty around this conclusion exists, highlighting the need for further research.

A cluster-randomised quality improvement study to improve two inpatient stroke quality indicators
Conclusion Quality improvement training was associated with early DVT improvement, but the effect was not sustained over time and was not seen with dysphagia screening. External quality improvement programmes may quickly boost performance but their effect may vary by indicator and may not sustain over time.

Topic identification for cross-sectoral quality assurance in stroke and TIA treatment.
Authors: Meyer S, Willms G, Broge B, Szecsenyi J Abstract The development of cross-sectoral quality assurance programs usually requires extensive topic identification. Illustrated by the complex processes of care for stroke and transient ischemic attacks (TIAs), a method for comprehensive topic identification is presented. The first step involves a thorough literature search in terms of systematic reviews, health technology assessments, guidelines, studies into healthcare delivery and the use of specific instruments. Routine data as well as epidemiologic studies are used to analyze the reality of service provision....

Cost-effectiveness analysis of tissue plasminogen activator in acute ischemic stroke in Iran.
CONCLUSIONS: The balance of hospitalization and rehabilitation costs and QALYs support the conclusion that treatment with intravenous tPA in the 4.5 hour time window is cost-effective from the perspectives of third party payer and inclusion of tPA in the insurance benefit package is reasonable. Economic burden of stroke: a systematic review on post-stroke care
ConclusionThe highest PSC costs were observed in the USA, with rehabilitation services being the main cost driver. Due to diversity in reporting, it was not possible to conduct a detailed cost analysis addressing different segments of services. Further approaches should benefit from the advantages of administrative and claims data, focusing on inpatient/outpatient PSC cost and its predictors, assuring appropriate resource allocation.

The weekend effect revisited: evidence from the Upper Austrian stroke registry
AbstractEmpirical evidence on the so-called ‘weekend-effect’ on stroke mortality is mixed with some studies reporting significantly higher mortality for weekend admissions and others finding no difference. The aim of this paper is to enhance the evidence on the weekend-effect on stroke mortality using a rich stroke registry data set from Upper Austria and to discuss underlying reasons for the heterogeneity in results. Using logistic regressions and ordinary least squares regressions with hospital and year-fixed effects, the outcomes of weekend versus weekday admissions are compared for patients admitted to 16 hospitals...
