A 74 year–old woman presented in December 2011 with left sided cerebellar symptoms. She was initially diagnosed as having a posterior circulation infarction and discharged from hospital on appropriate medication. However, over the following month she continued to deteriorate, developing increasing unsteadiness, falls, nausea and vomiting. A Case Of Hemophagocytic Lymphohistiocytosis Syndrome In A Patient With Multiple Sclerosis On Fingolimod Therapy. (P2.206)
CONCLUSIONS:As far as we know, this syndrome has never been reported in fingolimod treated patients. EBV reactivation may have been the triggering factor of HLH in our patient. Whether fingolimod contributed to HLH, through a direct mechanism, or indirectly favoring EBV reactivation, remains unknown. Our case highlights the diagnostic challenge of HLH and the need of monitoring and reporting serious adverse events in patients exposed to new IMD. Light chain amyloidosis: Experience in a tertiary hospital: 2005–2013
Conclusions Cardiac involvement is the major determinant of prognosis. Yield of involved organ biopsy is high (100% heart biopsies). Antineoplastic treatment with bortezomib and/or autologous bone marrow transplantation achieves hematological responses with improvements in organ impairment.

Progressive multifocal leukoencephalopathy during fumarate monotherapy of psoriasis

Clinical Relevance of Clonal Hematopoiesis in the Oldest-Old Population: Analysis of the "Health and Anemia" Study
Conclusion. Clonal hematopoiesis was associated with reduced survival in an oldest-old population. Specific mutational profiles define different risks of developing MDS and inflammatory/vascular diseases. Case Report: Interindividual variability and possible role of heterozygous variants in a family with deficiency of adenosine deaminase 2: are all heterozygous born equals?
In conclusion, this familiar case demonstrated the wide range of phenotypic variability in DADA2. The search for ADA2 mutations and the assessment of ADA2 activity should be considered also in patients with the association of hypogammaglobulinemia and inflammatory conditions, also with late presentation and in absence of vasculitis. Furthermore, the clinical picture of the deceased carrier suggests a possible contribution of heterozygous pathogenic variants to inflammation.
