Commentary: Until we take it seriously, the status quo of postoperative atrial fibrillation management will prevail
New-onset postoperative atrial fibrillation (POAF) is common following cardiac surgery, developing in almost one-third of patients after coronary artery bypass grafting (CABG) and two-thirds of patients after valvular surgery.1 The long-standing characterization of atrial fibrillation (AF) as a transient and benign postoperative complication may be an underlying reason for the relatively limited progress in terms of its prophylaxis and management.2 This assumption has only recently been called into question by  new studies reporting varying degrees of association between POAF and mortality, stroke, myocardial infarction, or renal failure.

ConclusionsPostoperative complications after cardiac surgery significantly impact outcomes that extend beyond the postoperative period. Stroke, renal failure, and pneumonia are particularly associated with poor long ‐term survival.

Synchronous versus staged carotid artery stenting and coronary artery bypass graft for patients with concomitant severe coronary and carotid artery stenosis: A systematic review and meta-analysis.
CONCLUSION: The simultaneous approach was associated with an increased risk of 30-day stroke compared to staged CAS and CABG. However, no statistically significant difference was found in long-term results of mortality, MI and stroke between the two approaches. Future studies are warranted to validate our results. Impact of Left Atrial Appendage Exclusion on Short Term Outcomes In Isolated Coronary Artery Bypass Graft Surgery.
Conclusions: LAA exclusion during isolated CABG in patients with AF is associated with a higher rate of 30-day readmission. Post-operative measures to mitigate the loss of the hormonal and hemodynamic effects of the LAA may increase the therapeutic benefit of this procedure. Association of Postoperative Complications and Outcomes Following Coronary Artery Bypass Grafting
ConclusionsPostoperative complications are associated with an increased risk of both early and late mortality and all-cause rehospitalization, particularly during the “value” window within 90 days of CABG. These findings underscore the need to develop avoidance strategies as well as cost-adjustment methods for each of these complications.
