Plasma exchange-A useful adjunct therapy to red cell exchange in patients with sickle cell disease and multiorgan dysfunction
CONCLUSION: The findings suggest TPE may be considered as an adjunct treatment for patients with acute complications of SCD that progress to MODS, especially in cases where there is no significant improvement following RBCx.PMID:37317564 | DOI:10.1111/trf.17448

Barriers and facilitators for adherence to direct oral anticoagulants in patients with atrial fibrillation: A qualitative approach
Direct oral anticoagulants (DOACs) are the cornerstone for stroke prevention in patients with atrial fibrillation (AF) [1]. DOAC non-adherence and discontinuation have been associated with higher thromboembolic and mortality risks [2]. Until now, studies trying to optimise adherence to DOACs show disappointing results with limited effectiveness of interventions [3]. An explanation might be that medication (non-)adherence is a very complex behaviour in which different determinants, varying between patients, play a role.

The potential role of micro-RNA 125b-5p level in predicting outcome from thrombolytic therapy in patients with acute ischemic stroke
AbstractSeveral studies highlighted a significant role of specific miRNA as diagnostic and prognostic biomarkers for acute ischemic stroke. The aim of this work was to study micro-RNA 125b-5p level in patients with acute ischemic stroke in relation to stroke etiology, risk factors, severity and outcome. This case –control study was conducted on 40 patients with acute ischemic stroke eligible for receiving rt-PA and 40 age and sex matched healthy controls, Patients were submitted to neurological and radiological assessment. Functional outcome after 3 months was assessed using the modified Rankin Scale (mRS ). Plasma micr...

Coagulation biomarkers for ischemic stroke
We report on coagulation biomarker-induced risk of IS, stroke severity, and outcomes following IS derived from prospective population studies, case-control studies, and acute-phase IS studies. We found indications that many coagulation and inflammation biomarkers are associated with IS, but it is early to conclude that any of these biomarkers can be applied in a therapeutic setting to predict patients at risk of IS, stroke severity at the acute phase, and clinical outcome after treatment. The strongest evidence for a role in IS was found for beta-thromboglobulin, von Willebrand factor, factor VIII, fibrinogen, thrombin-act...

Poststroke venous thromboembolism and neutrophil activation: an illustrated review
Res Pract Thromb Haemost. 2023 Apr 29;7(4):100170. doi: 10.1016/j.rpth.2023.100170. eCollection 2023 May.ABSTRACTPatients with acute ischemic stroke are at a high risk of venous thromboembolism (VTE), such as deep vein thrombosis (DVT), estimated to affect approximately 80,000 patients with stroke each year in the United States. The prevalence of symptomatic DVT after acute stroke is approximately 10%. VTE is associated with increased rates of in-hospital death and disability, with higher prevalence of in-hospital complications and increased 1-year mortality in patients with stroke. Current guidelines recommend the use of ...

Cerebral infarction and in-hospital mortality for patients admitted to hospital with intracardiac thrombus: insights from the National Inpatient Sample
Abstract The factors associated with cerebral infarction and mortality in patients hospitalized with intracardiac thrombus are unknown. A retrospective cohort study was undertaken of nationally representative hospital admissions in the National Inpatient Sample with a diagnosis of intracardiac thrombus between 2016 to 2019. Multiple logistic regressions were used to define factors associated with cerebral infarction and in-hospital mortality. There were a total of 175,370 admissions for patients with intracardiac thrombus and 10.1% patients had cerebral infarction (n  = 17,675). Intracardiac thrombus represented 4.4%...

Thrombotic Complications in Children with COVID-19 and MIS-C
J Thromb Haemost. 2023 May 31:S1538-7836(23)00434-8. doi: 10.1016/j.jtha.2023.05.020. Online ahead of print.ABSTRACTCoronavirus disease 2019 (COVID-19) associated coagulopathy is multifactorial and involves inflammation driven hypercoagulability, endothelial dysfunction, platelet activation and impaired fibrinolysis. Hospitalized adults with COVID-19 are at an increased risk of both venous thrombo-embolism (VTE) and ischemic stroke, resulting in adverse outcomes including mortality. While children with COVID-19 follow a less severe course, both arterial and venous thrombosis have been reported in hospitalized children with...

Clinical effectiveness and safety of edoxaban in obese patients — Results of the prospective Dresden NOAC Registry (NCT01588119)
Edoxaban is a direct factor Xa inhibitors approved for stroke prevention in atrial fibrillation (SPAF) and for venous thromboembolism (VTE) treatment. Edoxaban is given in a fixed dosing regimen (with recommended dose reductions for patients with renal impairment or body weight

von Willebrand factor, ADAMTS-13, and thrombospondin 1 in relation to clinical outcomes in elderly patients with a recent myocardial infarction
CONCLUSION: Although low ADAMTS-13 predicted death, the cardiovascular risk associated with VWF and ADAMTS-13 was weaker than previously reported. Low VWF is associated with new-onset AF and needs further research.PMID:37255854 | PMC:PMC10225923 | DOI:10.1016/j.rpth.2023.100164
