Transplants give Garden Plain man a second chance at life
Chris Simon’s bucket list wasn’t anything fancy. He simply wanted to be able to ride a bike again. Or take a swim. Or go camping with friends. “Stuff I used to do all the time,” Simon said. “And all of a sudden it was taken away.” Those once simple joys were stolen by cystic fibrosis, the same disease that claimed his younger sister Emily’s life in 2009. The Simon family on Sunday will join millions of others around the world in celebrating Easter, a day symbolizing new beginnings. The holiday will have special significance for Chris, 26, who has another chapter dawning in his life after a double-lung/liver t...

Evaluation of cardiac output by bioreactance technique in patients undergoing liver transplantation.
CONCLUSIONS: NICOM provided a comparable CI and SVI trend when compared to the gold standard PiCCO, but it raises concerns as an effective CO monitor because of its tendency to overestimate CI and SVI especially during the state of high cardiac output. Evaluation of pleth variability index as a predictor of fluid responsiveness during orthotopic liver transplantation
This study was designed to examine whether the PVI can reliably predict fluid responsiveness in OLT and to compare PVI with other hemodynamic indexes that are measured using the PiCCO2 monitoring system. Twenty-five patients were enrolled in this study. Each patient was monitored using the noninvasive Masimo and PiCCO2 monitoring system. PVI was obtained with a Masimo pulse oximeter. Cardiac index was obtained using a transpulmonary thermodilution technique (CITPTD). Stroke volume variation (SVV), pulse pressure variation, and systemic vascular resistance index were measured using the PiCCO2 system. Fluid loading (10 mL/k...

HDACi Valproic Acid (VPA) and Suberoylanilide Hydroxamic Acid (SAHA) Delay but Fail to Protect against Warm Hepatic Ischemia-Reperfusion Injury
ConclusionsVPA and SAHA delay warm hepatic I/R injury at least in part through modulation of SAPK-activation. However, these HDACi fail to exert organoprotective effects, in this setting. For VPA, belated damage is even aggravated.

Transmission of Balamuthia mandrillaris by Organ Transplantation
Conclusions. Balamuthia can be transmitted through organ transplantation with an observed incubation time of 17–24 days. Clinicians should be aware of Balamuthia as a cause of encephalitis with high rate of fatality, and should notify public health departments and evaluate transplant recipients from donors with signs of possible encephalitis to facilitate early diagnosis and targeted treatment. Organ procurement organizations and transplant centers should be aware of the potential for Balamuthia infection in donors with possible encephalitis and also assess donors carefully for signs of neurologic infection that...

The Role of the Model of End-Stage Liver Disease Score in Predicting Outcomes of Carotid Endarterectomy
Conclusion: High MELD score places asymptomatic patients undergoing carotid endarterectomy at a higher risk of adverse outcomes in the 30 days following surgery. This provides further empirical evidence for risk stratification when considering treatment for these patients. Outcomes of medical management or carotid stenting should be investigated in high-risk patients.

Chronic hepatothorax due to right diaphragmatic rupture: an anesthetic challenge in a rare case
Conclusions Hepatothorax is a rare condition and its repair may represent an anesthetic challenge. After liver replacement in the abdominal cavity during corrective surgery under general anesthesia complications may occur, particularly associated with pulmonary re-expansion. Effective teamwork and careful planning of surgery, between the surgical and anesthetic teams, are the key to success.

Outcomes in patients with diabetes 10 years after liver transplantation
ConclusionsIn our study, diabetes was associated with a higher risk of liver graft rejection and cardiovascular events. There was also a trend to higher mortality, although the effect was not statistically significant. These findings suggest that patients with diabetes require a more rigorous pretransplant evaluation and closer monitoring after transplantation, in order to try to reduce associated complications.

Hussain, John Hearty, Stuart Fairclough, Lynne Boddy, Kelly Mackintosh, Gareth Stratton, Nicky Ridgers, Dhiya Al-Jumeily, Ahmed J. Aljaaf, Jenet Lunn The growing trend of obesity and overweight worldwide has reached epidemic proportions with one third of the global population now considered obese. This is having a significant medical impact on children and adults who are at risk of developing osteoarthritis, coronary heart disease and stroke, type 2 diabetes, cancers, respiratory problems, and non-alcoholic fatty liver disease...

Autosomal dominant polycystic kidney disease: Study of clinical characteristics in an Indian population.
Abstract Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary form of kidney disease. Clinical data on this multisystem disorder are scarce from developing countries. We conducted a prospective observational study of the clinical profile of ADPKD patients at a single center over a period of six years. A total of 208 patients were studied. Majority were male (60.6%) and the mean age was 45.8 ± 14.5 years. About 61.5% had early stage (Stages 1-3) of chronic kidney disease (CKD) and 38.5% had advanced CKD (Stages 4 and 5). Clinical features observed included pain abdomen (46.2%), noctur...

What Causes Hyperammonemia?
Discussion Reye’s syndrome (RS)is named for Dr. Douglas Reye who along with Drs. G. Morgan and J. Baral described encephalopathy and fatty accumulation and degeneration in children in a 1963 Lancet article. RS usually affects children but can occur at all ages. All organs can be affected but the liver and brain are primarily affected causing liver failure and encephalopathy as toxic metabolites (especially ammonia) accumulate, and intracranial hypertension and cerebral edema occurs. As the ammonia levels begin to rise (> 100 mg/dL) patients lose their appetite, have nausea and emesis and mental status changes whic...

How to Boost Your Stem Cells
A California man named Kris Boesen was completely paralyzed after breaking his neck in a terrible car crash last March. But now he can brush his teeth and hug his family again… He's even started to regain sensation in his legs. And it's all because of a simple procedure he underwent about a month after his accident: stem cell therapy. During Kris' procedure, researchers at the University of Southern California injected 10 million stem cells into his spinal cord. Within two weeks, he could wiggle his fingers. Three months later, he was able to feed himself, write his name and operate his wheelchair. His re...

Prognostic value of perioperative assessment of plasma cardiac troponin I in patients undergoing liver transplantation.
CONCLUSION: value of cTnI level assessed 24 hours post-surgery was a reliable predictor of death following LTx with optimal cut-off value of 0.215 ng/mL. The surgery time was the most important predictor of cTnI elevation. PMID: 28455997 [PubMed - as supplied by publisher]
