Sildenafil Reduces Risk of Ischemic Stroke and Pump Thrombosis with Ongoing Low Level Hemolysis During Heart Mate II Support
Low level hemolysis after Heart Mate (HM) II implantation, as evident by an elevated discharge lactate dehydrogenase (LDH) level>400 U/L, is linked to subsequent thrombotic events. Free hemoglobin from ongoing hemolysis scavenges nitric oxide (NO) to create a NO deficiency which can augment platelet function. We hypothesized the potentiation of NO signaling during low level hemolysis with a phosphodiesterase-5 inhibitor, sildenafil, may lead to reduced thrombotic events.

The Treatment of Patients with Advanced Heart Failure Ineligible for Cardiac Transplantation with the HeartWare Ventricular Assist Device: Results of the ENDURANCE Supplement Trial
The ENDURANCE Trial previously demonstrated non-inferiority of the HeartWare centrifugal flow ventricular assist device system (HVAD) to the control HeartMate II (HMII) axial flow device in 445 end-stage heart failure patients ineligible for heart transplantation. However, the rate of strokes in the HVAD cohort was higher than expected. Analysis of early clinical trial data found that elevated mean arterial blood pressure (MAP) was a highly statistically significant independent risk factor for stroke events.

The Prognostic Value of Follow-up Hemodynamic Variables After Initial Management in Pulmonary Arterial Hypertension.
Conclusions -SVI and RAP were the hemodynamic variables independently associated with death or lung transplantation at first follow-up RHC after initial PAH treatment. The Impact of Age, Sex, Therapeutic Intent, Race, and Severity of Advanced Heart Failure on Short-term Principal Outcomes in the MOMENTUM 3 Trial
Primary outcomes analysis of the MOMENTUM 3 trial short term cohort demonstrated a higher survival rate free of debilitating stroke and reoperation to replace/remove the device (primary endpoint) in patients receiving the HeartMate 3 (HM3) compared to the HeartMate (HMII). We sought to evaluate the individual and interactive effects of pre-specified patient subgroups (age, sex, race, therapeutic intent (BTT/BTC/DT) and severity of illness) on primary endpoint outcomes in MOMENTUM 3 patients implanted with HM3 and HMII.

Comparison of a portable device versus a specific spirometer for cough flow testing
Conclusions: In this evaluation, including patients with heterogeneous disorders and normal controls, CPF readings were on average significantly higher when measured using the equipment with the CPF testing dedicated software; moreover this device allows for a better assessment of the cough maneuvers including visualization of the cough flow-volume loops.
