Blood transfusion for preventing primary and secondary stroke in people with sickle cell disease.
CONCLUSIONS: The STOP trial demonstrated a significantly reduced risk of stroke in participants with abnormal transcranial Doppler ultrasonography velocities receiving regular blood transfusions. The follow-up trial (STOP 2) indicated that individuals may revert to former risk status if transfusion is discontinued. The degree of risk must be balanced against the burden of chronic transfusions. The combination of hydroxyurea and phlebotomy is not as effective as "standard" transfusion and chelation in preventing secondary stroke and iron overload. Haemodilution for acute ischaemic stroke.
CONCLUSIONS: The overall results of this review showed no clear evidence of benefit of haemodilution therapy for acute ischaemic stroke.These results are compatible with no persuasive beneficial evidence of haemodilution therapy for acute ischaemic stroke. Recurrent right middle cerebral artery territory infarctions in a patient with JAK2 (V617F) point mutation positive polycythemia rubra vera (P2.258)
CONCLUSIONS: Hyperviscosity in polycythemia with hematocrit of more than 43[percnt] is known to cause ischemic events and it is evident in our case where our patient had recurrent ischemic strokes whenever the HCT is about 50[percnt] and the right MCA M1 segment though a mild stenosis, it becomes a critical factor for this patient as the patient had recurrent right MCA territory infarcts and hence in addition to regular phlebotomy to keep hematocrit less than 43[percnt], our patient might also need intervention in the form of cerebral angioplasty or stenting to treat even mild arterial stenosis. Pain and other non‐neurological adverse events in children with sickle cell anemia and previous stroke who received hydroxyurea and phlebotomy or chronic transfusions and chelation: Results from the SWiTCH clinical trial
Conclusions: In children with SCA and prior stroke, monthly transfusions and daily iron chelation provided superior protection against acute vaso‐occlusive pain SAEs when compared to hydroxyurea and monthly phlebotomy.

Educational Intervention Reduces Utilization of Low-Value Testing in Cerebrovascular Inpatients: An Example of Rapidly Implementable Quality Improvement (P2.309)
Conclusions: In this prospective quality improvement study, an educational intervention resulted in significant reduction in utilization of low-value tests in patients admitted to an inpatient cerebrovascular service. Validating Laboratory Results in a National Observational Cohort Study without Field Centers: the Reasons for Geographic and Racial Disparities in Stroke Cohort.
CONCLUSIONS: Simulation of non-optimal field processing and shipment to a central laboratory showed high variability in analytes released by platelets. The majority of other analytes produced valid results, but platelet contamination in REGARDS samples makes measurement of platelet proteins unadvisable in these samples. A Noninvasive Stroke Volume Monitoring for Early Detection of Minimal Blood Loss: A Pilot Study
Conclusion: Continuous noninvasive monitoring of SV may be superior to conventional indices (e.g., heart rate, blood pressure, or shock index) for early identification of acute blood loss. As an operator-independent and point-of-care technology, the SV whole body bio-impedance measurement may assist in accurate monitoring of potentially bleeding patients and early identification of hemorrhage.

Interventions for preventing silent cerebral infarcts in people with sickle cell disease.
CONCLUSIONS: We identified no trials for preventing silent cerebral infarcts in adults, or in children who do not have HbSS SCD.Long-term red blood cell transfusions may reduce the incidence of silent cerebral infarcts in children with abnormal TCD velocities, but may have little or no effect on children with normal TCD velocities.
