Collaborative Interventions Reduce Time-to-Thrombolysis for Acute Ischemic Stroke in an Urban, Public Hospital (P4.270)
Conclusions:Targeted, multidisciplinary, collaborative interventions are associated with substantial and significant reductions in time to thrombolysis. Diagnosis of DWI-negative acute ischemic stroke: A meta-analysis
Conclusions: A small but significant percentage of patients with AIS have a negative DWI scan. Patients with neurologic deficits consistent with posterior circulation ischemia have 5 times the odds of having a negative DWI scan compared to patients with anterior circulation ischemia. AIS remains a clinical diagnosis and urgent reperfusion therapy should be considered even when an initial DWI scan is negative.

Oxidative Stress Biomarkers of Brain Damage Clinical Sciences
Conclusions—Elevated hyperacute plasma F2-isoP concentrations independently predict the occurrence of IG and IG volume in patients with acute ischemic stroke. If validated in future studies, measuring plasma F2-isoP might be helpful in the acute setting to stratify patients with acute ischemic stroke for relative severity of ischemic injury and expected progression.

Effects of Noninvasive Brain Stimulation on Language Networks and Recovery in Early Poststroke Aphasia Clinical Sciences
Conclusions— Ten sessions of inhibitory rTMS over the right posterior inferior frontal gyrus, in combination with speech and language therapy, significantly improve language recovery in subacute ischemic stroke and favor recruitment of left-hemispheric language networks.

Quality Improvement Project: Improving the Time To Treatment in Inpatient Acute Ischemic Stroke (P7.130)
Conclusion/Proposal: In the inpatient setting there was a significant delay in the time to CT-scan and to t-PA; the main source of delay was time to CT-scan. There are many possible reasons for the delay: location of the CT-scanner, unfamiliarity with strokes by non-neurology staff, ready availability of t-PA. We proposed to target house staff education and t-PA availability. We developed a "stroke-code" checklist to be distributed to house staff from different services after a small explanatory lecture. A SMARTer approach to stroke care
Time is critical when it comes to stroke: early treatment is associated with better outcomes. According to the Screening with MRI for Accurate and Rapid stroke Treatment (SMART) study, small changes in quality improvement procedures enabled clinicians to use MRI scans to diagnose stroke patients before giving acute treatment, within 60 minutes of hospital arrival. MRI scans provide detailed images but take longer to complete than CT scans, which are commonly used in most centers.

Network dysfunction predicts speech production after left hemisphere stroke
Conclusions: Speech production is dependent on complex interactions among widely distributed brain networks, indicating that residual speech production after stroke depends on more than the restoration of local domain-specific functions. Our understanding of the recovery of function following focal lesions is not adequately captured by consideration of ipsilesional or contralesional brain regions taking over lost domain-specific functions, but is perhaps best considered as the interaction between what remains of domain-specific networks and domain-general systems that regulate behavior.

RehAtt - scanning training for neglect enhanced by multi-sensory stimulation in Virtual Reality.
CONCLUSION: RehAtt™ is a new concept for rehabilitation of neglect. Training with the VR-method improved spatial attention and showed transfer to improved spatial attention in activities of daily living in chronic neglect. How Patient Demographics, Imaging, and Beliefs Influence Tissue-Type Plasminogen Activator Use: A Survey of North American Neurologists Clinical Sciences
Conclusions— Diagnostic certainty and likelihood of treatment with tissue-type plasminogen activator increase with additional clinical data, with the history being the most important factor for diagnostic and treatment decisions. Respondents had difficulty in interpreting the results of CT perfusion scans although they had little impact on treatment decisions. We did not identify treatment bias based on patient age, race, or sex.

Continuous theta-burst stimulation may improve visuospatial neglect via modulating the attention network: a randomized controlled study.
CONCLUSION: cTBS of the left posterior parietal cortex in patients with VSN may induce changes in inter-regional RSFC in the right ventral attention network. These changes may be associated with improved recovery of behavioral deficits after behavioral training. The TPJ and superior temporal sulcus may play crucial roles in recovery from VSN. Lacunar Infarcts, but Not Perivascular Spaces, Are Predictors of Cognitive Decline in Cerebral Small-Vessel Disease Clinical Sciences
Conclusions—Although a feature of small-vessel disease, PvS are not a predictor of cognitive decline, in contrast to lacunes. This study highlights the importance of carefully differentiating between lacunes and PvS in studies investigating vascular cognitive impairment.
