A simple score to predict atherosclerotic or embolic intracranial large-vessel occlusion stroke before endovascular treatment

Early versus late start of direct oral anticoagulants after acute ischaemic stroke linked to atrial fibrillation: an observational study and individual patient data pooled analysis
Conclusions Our results do not corroborate concerns that an early DOAC-start might excessively increase the risk of ICH. The sevenfold higher risk of recurrent AIS than ICH suggests that an early DOAC-start might be reasonable, supporting enrolment into randomised trials comparing an early versus late DOAC-start.

Cerebral hyperperfusion syndrome after mechanical thrombectomy
Case summary A male patient in his late 50s who had untreated atrial fibrillation received mechanical thrombectomy for right middle cerebral artery occlusion (MCA), with complete recanalization. After initial neurological improvement, he developed headache and altered vigilance 2 days post-thrombectomy. Differential effect of mechanical thrombectomy and intravenous thrombolysis in atrial fibrillation associated stroke
Conclusions In patients treated with MT, comorbid AF is associated with faster procedural time, fewer passes, and increased rates of first pass success without increased risk of intracranial hemorrhage or worse functional outcomes. These results are in contrast to the increased hemorrhage rates and worse functional outcomes observed in AF associated stroke treated with supportive care and or IVT. These data suggest that MT negates the AF penalty in ischemic stroke.

In-thrombus thrombin secretion: a new diagnostic marker of atrial fibrillation in cryptogenic stroke
Conclusions An assay measuring the change in thrombin in clots retrieved during acute stroke endovascular thrombectomy procedures may serve as a diagnostic marker of the origin of the clot. The suggested mechanism for these differences may be the clot location before its retrieval, with high blood flow causing thrombin washout in atherosclerotic clots, in contrast to atrium appendage low blood flow retaining high thrombin levels.
