Teaching NeuroImages: Concurrent hemifacial spasm and trigeminal neuralgia due to vertebrobasilar dolichoectasia
A 64-year-old man reported a 2-week worsening of a 2-year history of paroxysmal, lancinating right facial pain and spasms. Head CT demonstrated a tortuous and ectatic vertebral-basilar system. MRI confirmed a dolichoectatic basilar artery that distorted the right trigeminal (figure 1) and facial nerve roots (figure 2). He failed conservative medical management including carbamazepine, oxcarbazepine, and levetiracetam. He underwent Gamma Knife radiosurgery with few months of pain relief and was offered botulinum neurotoxin injection for spasms.

An Innovative Community Geriatrics Elective to Teach Year 4 Medical Students About Nursing Home Care
In Singapore, Geriatrics is core curriculum in medical undergraduate training. Year two medical students are introduced to aging in a four hour program based in a nursing home. In their final year they spend one month in geriatric medicine as part of their 3 month internal medicine posting. In the fourth year there is an elective period of six weeks where they choose postings that they would like to spend time to learn more about the subject. There are 65 NHs in Singapore with 9495 beds as compared to 6 acute hospitals with 7500 beds. The NHs have more patients for students to learn from. The residents are also more stable...

Teaching NeuroImages: Ocular bruit in carotid-cavernous sinus fistula
A 57-year-old man who had a traffic accident 1 month previously presented with left ocular pain, double vision, and left eye proptosis with ptosis and conjunctival hemorrhage. Fundus showed dilated veins with no hemorrhages or disc edema. Left ocular motility showed complete external ophthalmoplegia (figure 1). Medical treatment of ureteral obstruction associated with ovarian remnants and/or endometriosis: report of 3 cases and review of the literature
Conclusions Low dose intermittent danazol or GnRH-a alone or with add-back, may be effective long-term therapies in endometriosis-associated ureteral obstruction when surgery is contraindicated, refused or difficult to perform.

The Evolution of an Inpatient Palliative Care Consultation Service in an Urban Teaching Hospital.
CONCLUSION: We describe the evolution of palliative care in a safety-net hospital. Medicine services which are largely resident run adopted early. Specialty services that are attending driven adopted later. We believe house staff and nurses were the initial change agents. The number of consultations increased when house staff and students began rotating on the service suggesting unmet demand due to the limited supply of providers.

In-hospital organization of primary care of patients presenting a life-threatening emergency: A French national survey in 32 university hospitals
Conclusion This survey is the first to assess the in-hospital organization of primary care for instable and life-threatening patients in France. Our results confirmed the extreme heterogeneity of structural and human organizations for primary in-hospital care of patients presenting at least one organ failure. Thus, a consensus is probably needed to homogenize and improve our practices.

Cohort of renal infarction during 2years at Grenoble teaching hospital.
CONCLUSION: Delay of diagnosis is a real problem for renal infarction, and need to be evocated every flank pain. LDH elevation may help clinician to suggest renal infarction and lead to CT scan. Association of delayed inflammatory syndrome, hypertension and hypokalemia after flank pain strongly suggest renal infarction.

Teaching NeuroImages: Carotid cavernous fistula in a patient with Ehlers-Danlos syndrome
A 25-year-old woman with Ehlers-Danlos syndrome type IV and history of spontaneous right petrous internal carotid artery dissection presents with severe headache, right eye pain with extraocular movements, proptosis, and chemosis of the right eye. She reported hearing her heartbeat in her right ear. CT angiogram (figure) revealed an engorged right ophthalmic vein. Subsequent digital subtraction angiography confirmed a carotid cavernous fistula along a previously dissected carotid artery.
