Recurrent migraine with binocular transient vision loss associated with acute stroke: A case report
CONCLUSION: Recurrent migraine with TBVL can be associated with acute stroke. It thus merits urgent evaluation and referral to the relevant department for a better outcome.PMID:34917346 | PMC:PMC8646124 | DOI:10.1016/j.amsu.2021.103062

E-040 Factors associated with failed mechanical thrombectomy in acute ischemic stroke
ConclusionFailed MT was encountered in approximately 12% of cases. The most common location of failed MT was intracranial and this was predominantly due to ICAS. Further studies to evaluate better treatment modalities of ICAS related LVO are warranted.Disclosures O. Lajthia: None. H. Ali: None. R. Neyes: None. E. Almallouhi: None. R. Chalhoub: None. K. Hidden Telltale Signs in Hyperacute Ischemic Stroke Caused by Aortic Dissection: A Case Report and Post Hoc Analysis
Conclusion: Hyperacute ischemic stroke caused by AD often hides some telltale signs. Clinicians should master basic clinical skills to exclude AD by looking for these telltale signs hidden in hyperacute ischemic stroke to avoid the fatal consequences of intravenous thrombolysis and/or cerebral angiography within the narrow window of time.

Dual potential embolic source detected in left atrium during hyperacute stroke
Clinical introduction A woman in her late 60s with a medical history of hypertension presented to the emergency department with symptoms of a stroke which began 40 min prior. Brain MRI was performed (figure 1A). Hyperacute CT angiography of the head and neck did not find any atherosclerosis. ECG is shown (figure 1B), and subsequent transthoracic echocardiography showed left atrium enlargement and left-to-right flow at the region of the fossa ovalis (figure 1C). Figure 1Brain MRI of diffusion-weighted image sequence (A), ECG (B), and transthoracic echocardiogram (C). Question Which of the following is the most likely mechan...

Geographic and Sex Difference in the Distribution of Intracranial Atherosclerosis in China Clinical Sciences
Conclusions— There exists geographic and sex difference in the distribution of symptomatic ICAS in China. Public health measures should strengthen improving social determinants of health and risk factor prevention/control in high-risk populations for decreasing stroke risk.

Recurrent Intracerebral Hemorrhage in Patients With Hypertension is Associated With APOE Gene Polymorphism: A Preliminary Study
This study evaluates the roles of apolipoprotein E (APOE) and α-1 antichymotrypsin (ACT) genes in patients with recurrent hypertensive ICH and compares patients with nonrecurring hypertensive ICH and normal controls. Methods: Thirty-three recurrent and 101 nonrecurrent patients with hypertension and ICH were included. The demographic, stroke risk factors, and computed tomographic or magnetic resonance imaging findings were recorded. Magnetic resonance angiography or digital subtraction angiography and vasculitic profile were done in recurrent group to exclude secondary causes of ICH. APOE and ACT gene polymorphisms were as...

Simultaneous onset of myocardial infarction and ischemic stroke in a patient with atrial fibrillation: Multiple territory injury revealed on angiography and magnetic resonance
An 84-year-old man with a history of hypertension and paroxysmal atrial fibrillation (AF) who received no anticoagulant drugs experienced acute chest pain and transient loss of consciousness. He was transferred to our emergency room. His initial electrocardiogram showed sinus rhythm with ST-segment elevation in the I, aVL, and V1–V6 leads. His blood pressure was 158/92mmHg and his pulse was regular at 70beats per minute. A chest radiograph showed increased heart size and pulmonary vascular congestion. His troponin T level was elevated (more than 0.1ng/mL). He was diagnosed with ST-elevation myocardial infarction (MI) and...

E-025 Aspiration thrombectomy of acute ischemic stroke patients with intracranial atherosclerotic disease or tandem lesion
ConclusionAspiration thrombectomy was effective in restoring blood flow and resulting in good 90-day functional outcome in AIS patients with anterior circulation tandem lesion, and the 90-day mortality rate in these patients was low. No significant difference in outcomes was detected between patients with a stent and patients without a stent.Disclosures O. Amusia: Unique Presentation of Stroke (P6.261)
CONCLUSIONS:Even though music and speech are similar in principle and execution they are functions of different hemispheres of the brain. Unlike aphasia, anatomical localization of amusia has not been established but it is known that right non dominant hemisphere is concerned with musical ability. Impairment of musical ability in the presence of intact speech can be the only presenting finding in stroke. Their identification is the key to early and appropriate treatment.Study Supported by:NoneDisclosure: Dr. Ramchandani has nothing to disclose. Dr. Alsmaan has nothing to disclose. Difference in Aneurysm Characteristics Between Ruptured and Unruptured Aneurysms in Patients With Multiple Intracranial Aneurysms Clinical Sciences
Conclusions— Aspect ratio ≥1.3 and irregular shape are associated with aneurysm rupture independent of aneurysm size and location, and independent of patient characteristics. Additional studies need to assess to what extent these factors increase the risks of rupture of small aneurysms in absolute terms.
