Factors Mediating Outcome After Stroke: Gender, Thrombolysis, and Their Interaction
AbstractSeveral studies, but not all, have shown that women benefit more from intravenous thrombolysis than men; few have accounted for pre-stroke mobility. Our aim was to determine whether there was an interaction between gender and thrombolysis treatment in 3-month modified Rankin Scale (mRS) score, after adjusting for pre-stroke mobility. We retrospectively reviewed medical records of 1390 consecutive ischemic stroke patients admitted between October 2012 and July 2015. The 3-month mRS was obtained from clinic visits. Thrombolysis-by-gender interaction was evaluated in univariate and multivariate analyses using ordinal ...

Stereotactic Catheter Ventriculocisternostomy for Clearance of Subarachnoid Hemorrhage Clinical Sciences
Conclusions—STX-VCS was feasible and safe in patients with severe aneurysmal subarachnoid hemorrhage. Initial results indicate that DCI and mortality can be reduced, and neurological outcome may be improved with this method.

Intravenous thrombolysis for patients with in ‐hospital stroke onset: propensity‐matched analysis from the SITS‐EAST registry
ConclusionsOur findings underline the safety and efficacy of IVT for IHS. They also underscore the potential of reducing in‐hospital delays for timely tPA delivery in patients with IHS.This article is protected by copyright. All rights reserved.

Impacts of Cigarette Smoking and Basilar Artery Flow on 1-year Recovery in 3-Month survivors of Intracranial Vertebrobasilar Artery Dissection-related Ischemic Stroke.
CONCLUSIONS: Our results suggest that adequate blood flow may be key to functional recovery after iVBDcaused PCS. Methods to improve blood flow and tissue perfusion after iVBD-caused PCS should be considered in the future clinical studies with the purpose to improve functional recovery in these patients. Elevated Red Cell Distribution Width is Associated with Cerebral Infarction in Aneurysmal Subarachnoid Hemorrhage
ConclusionsRDW elevation is associated with cerebral infarction and poor outcome after aSAH. Further evaluation of this association is warranted as it may shed light on mechanistic relations between anemia, inflammation, and thrombosis after aSAH.

Preexisting cognitive impairment in intracerebral hemorrhage
ConclusionsIn conclusion, cognitive impairment frequently precedes ICH. A higher frequency of cerebrovascular events suggests a role of vascular processes in the development of cognitive impairment before ICH.

The relationship of platelet-to-lymphocyte ratio with clinical outcome and final infarct core in acute ischemic stroke patients who have undergone endovascular therapy.
In this study we aimed to evaluate whether the PLR had a prognostic role in stroke patients undergoing thrombectomy and attempted to determine the effect that this ratio had on their survival. METHODS: Over a three-year period, demographic, clinical, and angiographic findings of 57 consecutive patients with acute ischemic stroke who underwent mechanical thrombectomy were evaluated. RESULTS: The patients were divided into two groups on the basis of a PLR level cut-off value of 145 based on receiver operating characteristic (ROC) curve. Successful revascularization (mTICI 2b and 3) was achieved in 42 of 57 (73.7%) pa...

Association between Leukoaraiosis and Poor Outcome is not due to Reperfusion Inefficiency after Intravenous Thrombolysis
Abstract Leukoaraiosis (LA) is associated with structural and functional cerebrovascular impairment, which may compromise the capacity of ischemic tissue to maximize reperfusion after intravenous thrombolysis (IVT). We aimed to determine whether severe LA is correlated with reperfusion inefficiency, which contributes to infarct growth and poor functional outcome. We analyzed data from our consecutive acute ischemic stroke (AIS) patients who had acquired baseline and 24-h follow-up diffusion- and perfusion-weighted imaging. Reperfusion was defined as reduction of ≥70 % of hypoperfusion lesion at 24 h from basel...

Women have a poorer very long-term functional outcome after stroke among adults aged 18–50 years: the FUTURE study
In conclusion, 14 years after an ischemic cerebrovascular event in young adults, one out of five IS survivors and one out of ten TIA survivors is still dependent in daily life, with a two to threefold higher risk of a poor outcome in women. This includes a period of life, during which important decisions regarding work and family life have to be made.

The stroke east Saxony pilot project for organized post‐stroke care: a case–control study
ConclusionsOur pilot data suggest that organized post‐stroke care enhances achievement of secondary prevention goals. Its possible effect on stroke recurrence, long‐term disability, and quality of life is currently investigated in a prospective cohort study. The Stroke East Saxony Pilot Project for Organized Post‐Stroke Care was case–control study to explore the feasibility of a self‐developed standardized post‐stroke pathway and its impact on secondary stroke prevention and long‐term outcome in patients with acute stroke. These pilot data suggest that organized post‐stroke care enhances achievement of sec...

Severe Cerebral Venous and Sinus Thrombosis: Clinical Course, Imaging Correlates, and Prognosis
Conclusion The outcome of severe CVT is almost equally divided between severe impairment or death and survival with no or only mild handicap. Specifically, space-occupying mass effect and associated neurologic deterioration seem to determine a poor outcome. Therefore, early detection and treatment of mass effect should be the focus of critical care.

Thrombectomy assisted by carotid stenting in acute ischemic stroke management: benefits and harms
This study assesses the benefits and harms of thrombectomy assisted by carotid stenting and identifies factors associated with clinical outcome and procedural complications. Retrospective single-center analysis of 47 consecutive stroke patients with carotid occlusion or high-grade stenosis and concomitant intracranial embolus treated between September 2011 and December 2014. Benefits included early improvement of stroke severity (NIHSS ≥ 10) or complete remission within 72 h and favorable long-term outcome (mRS ≤ 2). Harms included complications during and following EVT. Mean age was 64.3 years (standard deviatio...

Influence of previous physical activity on the outcome of patients treated by thrombolytic therapy for stroke
Abstract Physical activity prevents stroke and is associated with less severe strokes. The neuroprotective effect in patients treated with intravenous (i.v.) recombinant tissue plasminogen activator (rt-PA), remains uncertain. We aimed at evaluating the relationship between previous physical activity and outcomes in stroke patients treated with i.v. rt-PA. OPHELIE-SPORT was a prospective observational multicenter study conducted in French and Japanese stroke patients treated with i.v. rt-PA. We evaluated the presence, weekly duration (<2, 2–5, >5 h) and intensity (light, moderate, heavy) of previous leisu...

Post‐stroke cognitive impairment is common even after successful clinical recovery
ConclusionsCognitive impairment as evaluated with a comprehensive neuropsychological assessment is prevalent in stroke survivors even with successful clinical recovery. Typically multiple domains and complex cognitive abilities are affected. MMSE is not sensitive in detecting these symptoms. Post‐stroke cognitive impairment is strongly related to poor functional outcome.
