A simple 8‐point score strongly predicts early outcomes in acute stroke
ConclusionsA simple 8‐point clinical score is highly predictive of acute stroke mortality and length of hospital stay. It could be used as prognostic tool in service planning and also to risk‐stratify patients to use these outcomes as markers of stroke care quality across institutions.

Outcomes for people with atrial fibrillation in an Australian national audit of stroke care
ConclusionsHospital processes of care can influence outcomes in stroke patients with atrial fibrillation. The greater in‐hospital mortality experienced by stroke patients with atrial fibrillation may be attenuated by admission to a stroke unit, and for ischemic stroke, early administration of aspirin.

Comparison between angiotensin‐converting enzyme inhibitors and angiotensin receptor blockers on the risk of stroke recurrence and longitudinal progression of white matter lesions and silent brain infarcts on MRI (CEREBRAL study): rationale, design, and methodology
ConclusionsOur study will clarify whether angiotensin‐converting enzyme inhibitors or angiotensin II receptor blockers are more effective for preventing primary and recurrence of ischemic stroke, including the progression of asymptomatic cerebral lesions on magnetic resonance imaging, in elderly hypertensive patients.

Apparent diffusion coefficient threshold for delineation of ischemic core
ConclusionsOur data suggest that the ischemic core can be identified with an absolute apparent diffusion coefficient threshold. This threshold can be implemented in image analysis software for fully automated segmentation of the ischemic core.

How much diffusion lesion reversal occurs after treatment within three‐hours of stroke onset?
In this issue of the journal, Sakamoto et al. present an analysis of diffusion lesion evolution in acute ischemic stroke patients first imaged less than three‐hours from stroke onset. Although temporary lesion volume reduction was observed following reperfusion, the affected regions were generally abnormal on follow‐up fluid attenuated inversion recovery imaging, indicating lack of tissue salvage. Enlarged perivascular spaces and cerebral small vessel disease
ConclusionsEnlarged perivascular spaces are associated with age, lacunar stroke subtype and white matter lesions and should be considered as another magnetic resonance imaging marker of cerebral small vessel disease. Further evaluation of enlarged perivascular spaces in studies of ageing, stroke, and dementia is needed to determine their pathophysiological importance.

Effect of addition of clopidogrel to aspirin on stroke incidence: Meta‐analysis of randomized trials
ConclusionsThis meta‐analysis demonstrates a substantial relative risk reduction in stroke by clopidogrel plus aspirin vs. aspirin alone that is consistent across different trial cohorts. Major haemorrhage is increased by dual antiplatelet therapy.
