Visual field defect after ischemic stroke —impact on mortality
ConclusionsHaving a visual field defect after ischemic stroke is independently associated with increased mortality. This should be addressed when selecting candidates for thrombolysis and in the rehabilitation process.

Safety of off‐label stroke treatment with tissue plasminogen activator
ConclusionsIntravenous thrombolysis with tPA may be safe and efficient in stroke patients with a number of formal contraindications to tPA. Prospective randomized trials are imperative to clarify the need for a re‐evaluation of the current contraindications to tPA.

Montreal Cognitive Assessment and Mini–Mental State Examination are both valid cognitive tools in stroke
ConclusionsThe MoCA is a valid screening tool for post‐stroke cognitive impairment; it is more sensitive but less specific than the MMSE. Contrary to the prevailing view, the MMSE also exhibited acceptable validity in this setting.

Association of uric acid and carotid artery disease in patients with ischemic stroke
ConclusionOur study demonstrated that higher uric acid level is strongly associated with CAD. Elevated uric acid might be injurious for large cerebral arteries with some probable confounding risk factors. Further prospective large clinical trials will determine whether lowering uric acid level reduces the frequency of CAD and ischemic stroke.

Mild stroke: safety and outcome in patients receiving thrombolysis
ConclusionsThis study highlights the efficacy of rt‐PA in patients with acute cerebral ischemia presenting with mild symptoms and confirms the low‐risk profile of this treatment.

Bone marrow mononuclear cell therapy in ischaemic stroke: a systematic review
Bone marrow mononuclear cell (BM‐MNC) therapy has emerged as a potential therapy for the treatment of stroke. We performed a systematic review of published studies using BM‐MNC therapy in patients with ischaemic stroke (IS). Literature was searched using MEDLINE, PubMed, EMBASE, Trip Database, Cochrane library and clinicaltrial.gov to identify studies on BM‐MNC therapy in IS till June, 2016. Data were extracted independently by two reviewers. STATA version 13 was used for carrying out meta‐analysis. We included non‐randomized open‐label, single‐arm and non‐randomized comparative studies or randomized contro...

Wake ‐up stroke—Amendable for thrombolysis‐like stroke with known onset time?
Patients suffering an acute ischemic stroke can be treated with intravenous thrombolysis in the absence of contraindications. A known onset time is a prerequisite as treatment, according to guidelines, has to be started within 4.5 hours. In patients awakening with a stroke, the last time they were seen without a neurological deficit is assumed to be the time of onset. Thus, despite of lack of contraindications on initial brain imaging, these patients are largely excluded from therapy. This review discusses the underlying pathophysiological, clinical, and radiological evidence surrounding wake‐up stroke and its consequen...

Prehospital pathways of occipital stroke patients with mainly visual symptoms
ConclusionsOccipital stroke patients with visual symptoms contact health care late, are inadequately recognized, and present with complex prehospital pathways. Consequently, they are often ineligible for IV thrombolysis. This presents a missed opportunity for preventing permanent visual field defects.

Non ‐persistence in ischaemic stroke: Risk of recurrent vascular events
ConclusionAfter a long follow‐up period, ischaemic stroke patients with a high vascular risk profile who stopped taking their secondary preventive medication had an increased risk of a recurrent vascular events.

Hydration prevents chronic hyperglycaemic patients from neurological deterioration post ‐ischaemic stroke
ConclusionsChronic hyperglycaemia was associated with the admission NIHSS score and neurological deterioration after excluding the effect of stress hyperglycaemia. Furthermore, hydration therapy may help prevent neurological deterioration.

CRP gene polymorphism predicts post‐stroke functional outcome in Han Chinese
ConclusionsOur study indicates that SNP rs1130864 in the CRP gene is an independent predictor of 3‐month functional outcome in patients with first‐onset IS in a Han Chinese population. Further studies in different ethnic groups are needed to validate our findings.
