Long-Term Risk of Acute Myocardial Infarction, Stroke, and Death With Outpatient Use of Clarithromycin: A Retrospective Cohort Study
AbstractIn a retrospective cohort study of patients enrolled in the UK Clinical Practice Research Datalink during 2000 –2013, we evaluated long-term risks of death, stroke, and acute myocardial infarction (AMI) in adults prescribed clarithromycin. Patients were outpatients aged 40–85 years, who were prescribed clarithromycin (n = 287,748), doxycycline (n = 267,729), or erythromycin (n = 442,999), orHelicobacter pylori eradication therapy with a proton pump inhibitor, amoxicillin, and either clarithromycin (n = 27,639) or metronidazole (n = 14,863). Association Between Proton Pump Inhibitor Use and Risk of Stroke
We read with interest the article by Nguyen et al1 evaluating the association between proton pump inhibitor (PPI) use and ischemic stroke. The authors found that PPI use was associated with a significant increase in the risk of ischemic stroke after adjustment for established risk factors for stroke including smoking status, body mass index, p hysical activity, dietary quality, alcohol use, menopausal hormone use in women, multivitamin use, regular aspirin use, regular nonaspirin nonsteroidal anti-inflammatory drug, use and personal history of hypertension, hyperlipidemia, coronary artery disease, or diabetes.

Does proton pump inhibitor reduce the antiaggregant efficacy of aspirin in ischemic stroke?
CONCLUSION: The combination of 30 mg lansoprazole and 100 mg aspirin does not cause a decrease in antiaggregant activity in the early period, but chronic use was not evaluated in this study. Patients with AR may benefit from an increase in the dose of aspirin.PMID:36474449
