Economic and Societal Aspects of Stroke Management
Stroke is a major health burden worldwide with attendant mortality, morbidity, and cost. In 2010, there were approximately 16.9 million strokes and an estimated 33 million stroke survivors worldwide. Also, in the United States, stroke is the third leading cause of death, with ischemic stroke resulting in 8% 30-day mortality (20% for hemorrhagic stroke). The staggering economic cost of the disease is driven largely by disability and long term care. Efforts in stroke healthcare delivery are focusing on performance, efficiency and value to better serve the consumer.

Discrepancies between current and ideal endovascular stroke treatment practice in Europe and North America: Results from UNMASK EVT, a multidisciplinary survey.
CONCLUSION: In the majority of North American states/provinces and European countries covered in this study, the discrepancy between endovascular therapy decisions under current local resources and assumed ideal conditions seems to be small, even in countries with a limited economic status and healthcare infrastructure. Comparative Outcomes of Mechanical Thrombectomy for Acute Ischemic Stroke in Patients  with and without Atrial Fibrillation
Stroke is a leading cause of disability and death in the United States (1). Cardioembolic strokes account for approximately 1 in 5 ischemic strokes, with atrial fibrillation (AF) being the most common cause of cardioembolic stroke (2). Acute ischemic strokes in patients with AF are associated with a higher degree of disability and worse outcomes than in patients without AF (3). Because of its proven efficacy, mechanical thrombectomy (MT) with second-generation stent retrievers has been recently assigned a class IA recommendation for the treatment of ischemic stroke in select group of patients (4).

Disparities in the Use of Mechanical Thrombectomy Alone Compared with Adjunctive Intravenous Thrombolysis in Acute Ischemic Stroke in the United States INTERVENTIONAL
CONCLUSIONS: Our results indicated that mechanical thrombectomy alone was used more frequently than MT+IV-tPA among patients with acute ischemic stroke. The disparity between those who identify as White and non-White persisted across the years, though it is closing.

The Role of Interventional Radiologists in Acute Ischemic Stroke Interventions: A Joint Position Statement from the Society of Interventional Radiology, the Cardiovascular and Interventional Radiology Society of Europe, and the Interventional Radiology Society of Australasia
Stroke is a major public health issue. Worldwide, the incidence of new strokes is 16.9 million per year (1). In the United States, the incidence of new strokes is 795,000 per year, of which 87% are ischemic (2). Endovascular thrombectomy (EVT) is proven to provide better clinical outcomes in patients with ischemic strokes caused by large-vessel occlusion compared with best medical therapy alone (3 –12). Furthermore, endovascular thrombectomy has proven to be cost-effective (13). The American Heart Association (AHA) and multiple international stroke organizations recommend EVT as standard of care for selected patients (14–18).

Refractory Stroke Thrombectomy: Prevalence, Etiology, and Adjunctive Treatment in a North American Cohort INTERVENTIONAL
CONCLUSIONS: Refractory stroke thrombectomy is encountered in <10% of cases, independently associated with diabetes, and related to underlying vessel wall pathology (intracranial atherosclerotic disease and/or intracranial arterial dissection) or, less commonly, recalcitrant emboli. Emergent salvage interventions with glycoprotein IIb/IIIa inhibitors or intracranial angioplasty and/or stenting are safe and effective adjunctive treatments.

Effect of Radiographic Contrast Media Shortage on Stroke Evaluation in the United States ADULT BRAIN
CONCLUSIONS: Our analysis reported changes in the use of CTA and CTP in patients with acute ischemic stroke during the contrast media shortage. Further research needs to identify effective strategies to reduce the reliance on contrast media–based studies such as CTA and CTP without compromising patient outcomes.
