Effects of healthcare system transformations spurred by the COVID-19 pandemic on management of stroke and STEMI: a registry-based cohort study in France
Conclusions The pandemic did not markedly alter the functioning of the emergency network. Although stroke patient management deteriorated, the resilience of the STEMI pathway was linked to its stronger structuring. Transversal reorganisations, aiming at concentrating resources on emergency care, contributed to maintenance of the quality of care. Trial registration number NCT04979208.

Theoretical modeling of spatial accessibility in the management of stroke in the Rhône department (France) and comparison with measured data
ConclusionTheoretical models were highly correlated and the correlation with the measured data was mostly correct. The few minutes difference between theoretical models and measured data could be explained by traffic hazards and organizational vagaries that always tend to disrupt modeling results. Using measured data was found to be very useful to perform theorical model and to develop a robust model of stroke transportation.

Outcomes of Acute Stroke Patients Requiring Mechanical Ventilation: Study Protocol for the SPICE Multicenter Prospective Observational Study
ConclusionsThe SPICE multicenter study will investigate 1-year outcomes, ethical issues, as well as care pathways of acute stroke patients requiring invasive ventilation in the ICU. Gathered data will delineate human resources and facilities needs for adequate management. The identification of prognostic factors at the acute phase will help to identify patients who may benefit from prolonged intensive care and rehabilitation.Trial registration: NCT03335995.

Emergency department admissions for myocardial infarction and stroke in France during the first wave of the COVID-19 pandemic: National temporal trends and regional disparities
CONCLUSIONS: The decrease in ED admissions for MI and stroke observed during the lockdown was probably caused by fear of COVID-19 and augmented by the lockdown, and was heterogeneous across the French territory. ED admissions were slow to return to the usual levels from previous years, without a compensatory increase. These results underline the need to reinforce messages directed at the population to encourage them to seek care without delay in case of cardiovascular symptoms.PMID:33893038 | DOI:10.1016/j.acvd.2021.01.006

Fluid Cognition Relates to Locomotor Switching in Neurotypical Adults, Not Individuals After Stroke
CONCLUSIONS: Individuals after stroke appear to have deficits in locomotor switching compared with neurotypical adults. The relationship between fluid cognition and locomotor switching was significant in neurotypical adults but not in individuals after stroke. Depressive Symptoms Moderate the Relationship among Physical Capacity, Balance Self-Efficacy, and Participation in People after Stroke
CONCLUSION: The results suggest the relationship between physical capacity and participation appears to be mediated by balance self-efficacy in individuals after stroke with low reports of depressive symptoms but in those with high reports of depressive symptoms, physical capacity and balance self-efficacy are unrelated to participation.IMPACT: Targeting balance self-efficacy to improve post stroke participation may only be beneficial for individuals with low reports of depression. In individuals after stroke with high reports of depression, treatment should include and emphasize the treatment of those depressive symptoms....

Large Vessel Occlusion in Patients With Minor Ischemic Stroke in a Population-Based Study. The Dijon Stroke Registry
Conclusion:Large vessel occlusion (LVO) in minor stroke is non-exceptional, and our findings highlight the need for emergency arterial imaging in any patients suspected of acute stroke, including those with minor symptoms because of the absence of obvious predictors of proximal LVO.
