Postoperative disability and one-year outcomes for patients suffering a stroke after carotid endarterectomy
Objective: Although post-carotid endarterectomy (CEA) strokes are rare, they can be devastating. The degree of disability that patients develop after such events and its effects on long-term outcomes are unclear. Our goal was to assess the extent of postoperative disability in patients suffering strokes after CEA and evaluate its association with long-term outcomes.

Features of gas exchange and metabolism of the brain during revascularization of the carotid arteries
Bratisl Lek Listy. 2023 Feb 28. doi: 10.4149/BLL_2023_054. Online ahead of print.ABSTRACTThe relevance of the stated topic of scientific research is determined by the fact that stroke is still the most important medical and social problem both in the world as a whole and in the Republic of Kazakhstan in particular due to high rates of morbidity, mortality, and disability. In addition, cerebrovascular diseases occupy one of the leading places in the structure of morbidity, disability and mortality, second only to coronary heart disease in Kazakhstan as well as around the world. The purpose of this research work is to study ...

Role of mini access during carotid endarterectomy: a prospective observational study
CONCLUSIONS: According to these results the mini skin incision allows a sufficient vessels exposure for dissection, endarterectomy, reconstruction of carotid artery and shunt positioning, minimizing surgical invasiveness, decreasing the incidence of temporary cervical nerve dysfunction and improving the aesthetic result with significative less pain suffered by the patients. Transverse cervical and great auricular nerves sparing decreased postoperative hypo-paresthesia in the neck, improving patient's satisfaction. These data suggest that this procedure can be considered safe. Exploratory analysis suggested that it could po...

Carotid atherosclerotic disease: A systematic review of pathogenesis and management
Brain Circ. 2022 Sep 21;8(3):127-136. doi: 10.4103/bc.bc_36_22. eCollection 2022 Jul-Sep.ABSTRACTCarotid stenosis is an important contributor to ischemic stroke risk with resultant significant impact on neurological disability and death in adults and with worldwide implications. Management of carotid stenosis is impacted by whether there are associated symptoms along with the degree of stenosis. Understanding of the pathogenesis of carotid atherosclerosis or stenosis is important in management of carotid stenosis. Atherosclerotic plaque formation is a chronic insidious process with a number of potential contributors to the...

Outcomes of transcarotid artery revascularization: A systematic review
CONCLUSION: TCAR is associated with a lower risk of perioperative stroke compared to TFCAS. TCAR is also associated with shorter operative time, lower risk of CNI and less blood loss compared to CEA. TCAR may be a promising treatment option besides TFCAS and CEA.PMID:36039496 | DOI:10.1177/15910199221123283

Postoperative Disability and One-Year Outcomes for Patients Experiencing Stroke After Carotid Endarterectomy
Although, post-carotid endarterectomy (CEA) stroke is rare, it can be devastating. The impairment degree and 1-year effects are unclear. Our goal was to assess the postoperative and 1-year outcomes for patients experiencing a stroke after CEA without preoperative disability.

Immediate and Long Term Results of a Randomised Single Centre Study of Surgical Treatment of Asymptomatic Stenosis of the Internal Carotid Arteries
Introduction: Cerebrovascular accident (CVA) is one of the main causes of disability and mortality in many developed countries of the world. The results of numerous studies have confirmed the positive effect of carotid endarterectomy (CEA) in the prevention of stroke development in both symptomatic and asymptomatic patients. Balloon angioplasty (BA) and carotid artery stenting (CAS) have evolved, in part, because of the need for alternative, less invasive treatments for patients considered poor candidates for surgery based on anatomical or clinical parameters.

A Stepwise Protocol to Reduce Haematoma Rates in Carotid Endarterectomy
Introduction: Stroke is the leading cause of long term disability in developed countries, with symptomatic carotid artery stenosis causing 25% of ischaemic strokes. Timely carotid endarterectomy (CEA) for significant stenosis reduces the risk of further strokes. Haematoma is a complication of CEA, especially in patients on dual antiplatelet therapy. Haematoma can result in stroke, death, and prolonged hospital stay. To date there has been limited research on protocols that can prevent this complication.

Case Report: Evaluating Biomechanical Risk Factors in Carotid Stenosis by Patient-Specific Fluid-Structural Interaction Biomechanical Analysis
Conclusions: Computational biomechanical analysis is a useful tool to provide the biomechanical risk factors to help clinicians assess and predict the patient-specific plaque vulnerability. The FSI computational model coupling the structural and hemodynamic computational analysis, better replicates the in vivo biomechanical condition, which can provide multiple structural and flow-based risk factors to assess plaque vulnerability.Cerebrovasc Dis

Patches of different types for carotid patch angioplasty.
CONCLUSIONS: The number of outcome events is too small to allow conclusions, and more trial data are required to establish whether any differences do exist. Nevertheless, there is little to no difference in effect on perioperative and long-term ipsilateral stroke between vein and any synthetic patch material. Some evidence indicates that other synthetic patches (e.g. PTFE) may be superior to Dacron grafts in terms of perioperative stroke and TIA rates, and both early and late arterial re-stenosis and occlusion. Pseudoaneurysm formation may be more common after use of a vein patch than after use of a synthetic patch. Bovin...
