Reperfusion Changes After  Stroke and Practical Approaches for Neuroprotection
Reperfusion is the first line of care in a growing number of eligible acute ischemic stroke patients. Early reperfusion with thrombolytic drugs and endovascular mechanical devices is associated with improved outcome and lower mortality rates compared with natural history. Reperfusion is not without risk, however, and may result in reperfusion injury, which manifests in hemorrhagic transformation, brain edema, infarct progression, and neurologic worsening. In this article, the functional and structural changes and underlying molecular mechanisms of ischemia and reperfusion are reviewed. The pathways that lead to reperfusion...

A back and forth manual aspiration technique using a SOFIA Plus catheter for acute ischemic stroke: technical note
ConclusionThis modified technique with back and forth movements of the SOFIA Plus catheter over the thrombus during aspiration may facilitate thrombectomy in AIS. However, further studies in larger groups are necessary to elucidate its efficacy and to compare with current techniques.

Paradoxical Thromboembolic Ischemic Stroke and Pulmonary Embolism after Uterine Fibroid Embolization
We report a case of acute paradoxical ischemic stroke after UFE in a patient with an inherited thrombophilia, who was successfully treated with endovascular clot retrieval. Institutional review board approval was not required.

Improved detection and characterization of arterial occlusion in acute ischemic stroke using Contrast Enhanced MRA
Conclusions:Our study showed that CEMRA imaging detected arterial occlusions better than TOF MRA in AIS patients and more precisely such that thrombus length and location could be known, which improves the patient’s management and care.

Predictors and Clinical Impact of Delayed Stent Thrombosis after Thrombectomy for Acute Stroke with Tandem Lesions INTERVENTIONAL
CONCLUSIONS: We observed a non-negligible rate of delayed stent thrombosis with significant negative impact on clinical outcome. Future studies should systematically measure and report stent patency rates.

Peri-thrombus vascular hyperintensity sign: detection of intracranial thrombus location and length in acute ischemic stroke
ConclusionThe PVHS on 3D BB contrast-enhanced MRI is a highly specific tool for evaluating the location and length of a thrombus in AIS patients.

Direct aspiration thrombectomy experience with the SOFIA 6F catheter in acute ischemic stroke
ConclusionIn the majority of our cases, the SOFIA 6F catheter provided effective and rapid recanalization with aspiration thrombectomy.

Concomitant AngioVac thrombectomy and patent foramen ovale closure in a patient with a large right atrial thrombus and recent paradoxical embolic stroke.
Abstract A 59-year-old male with a history of gallbladder adenocarcinoma receiving chemotherapy and on therapeutic anticoagulation for portal vein thrombosis presented to the emergency department via ambulance after being found unresponsive and in cardiac arrest. Initial workup upon return of spontaneous circulation revealed a large right atrial mass, patent foramen ovale (PFO), and bilateral acute cortical infarctions. This constellation of findings were concerning for PFO-related paradoxical embolic strokes. Given the risk of recurrent paradoxical embolic events and the absolute contraindication to thrombolysis ...

Association of thrombus density and endovascular treatment outcomes in patients with acute ischemic stroke due to M1 occlusions
ConclusionIn patients with M1 occlusions, thrombus density was not clearly associated with procedural and clinical outcomes after EVT.

A nomogram for predicting thrombus composition in stroke patients with large vessel occlusion: combination of thrombus density and perviousness with clinical features
ConclusionThis study has developed and internally validated an easy-to-use nomogram which can help predict clot composition and optimize therapeutic strategies for thrombectomy.

Identifying ex vivo acute ischemic stroke thrombus composition using electrochemical impedance spectroscopy
CONCLUSION: Combination of EIS and machine learning can reliably predict the RBC composition of retrieved ex vivo AIS thrombi and then classify them into groups according to their RBC composition with good sensitivity and specificity.PMID:37192738 | DOI:10.1177/15910199231175377
