Manual thrombectomy efficiency in relationship to the area at risk in patients with myocardial infarction with TIMI 0 ‐1 coronary flow: Insights from an all comers registry
ConclusionIn comparison with the group where MT was inefficient, efficient MT may modestly reduce final infarct size without a significant clinical benefit. In comparison with PCI‐only, no difference was found regarding stroke. © 2017 Wiley Periodicals, Inc.

A novel approach to extraction of a large thrombus on the intraventricular guide ‐wire during transcatheter aortic valve replacement
We report a case where a large guide‐wire thrombus identified during TAVR was successfully removed using a novel approach, preventing a potentially major stroke in this high‐risk patient. © 2015 Wiley Periodicals, Inc.

Single trans ‐septal access technique for left atrial intracardiac echocardiography to guide left atrial appendage closure
ConclusionsThis initial experience suggests that LAA occlusion with the Amplatzer Amulet device using ICE guidance from the left atrium via a single trans‐septal technique is feasible and safe.

The Efficacy and Safety of the WATCHMAN Device in LAA Occlusion in Patients with Non-Valvular Atrial Fibrillation Contraindicated to Oral Anticoagulation: A Focused Review.
Abstract The standard treatment for stroke risk patients with non-valvular atrial fibrillation (NVAF) is the use of oral anticoagulants (OACs). However, a substantial number of patients have relative or absolute contraindications to OACs due to concerns of major bleeding risk and other adverse effects while using oral anticoagulation therapy. Recently, occurrences of exclusion of the left atrial appendage (LAA) in patients with contraindication to anticoagulation therapy are widely expanding worldwide, causing major contentious discussions. The LAA is the commonest place of thrombus formation; therefore, the conce...

Radial versus femoral artery access in patients undergoing PCI for left main coronary artery disease: analysis from the EXCEL trial.
CONCLUSIONS: In the EXCEL trial, PCI of LM disease with TRA was associated with comparable early and late clinical outcomes as TFA. PMID: 30111521 [PubMed - as supplied by publisher]

Factors Associated with Prolonged Survival in Left Ventricular Assist Device Recipients
ConclusionsPreoperative diabetes and amiodarone use were associated with poor long-term survival in LVAD recipients. Development of early complications of stroke, gastrointestinal bleeding, hemolysis, or pump thrombosis was also associated with poor long-term survival. Early diagnosis and treatment of these complications may improve survival in LVAD recipients.

Effect of Hydraulic Expansion on Arteriovenous Fistula of Hemodialysis Patients
Conclusion Hydraulic expansion showed no difference from conventional surgery in complication after operation, and could decrease the time reliance on catheters and the risk of catheter-related infection, thrombosis, and decrease the related medical care costs. Reversal and Resumption of Antithrombotic Therapy in LVAD- Associated Intracranial Hemorrhage
ConclusionsReversal of anticoagulation appears safe following LVAD-associated intracranial hemorrhage, though inadequate reversal was common. Resumption of warfarin±antiplatelet was associated with fewer fatal and non-fatal thrombotic events compared to antiplatelets alone, though more non-fatal hemorrhage events occurred.

Commentary: Left ventricular size and left ventricular assist device support outcomes: Bigger is better?
Durable left ventricular assist device (LVAD) therapy affords patients significant improvements in survival, functional capacity, and quality of life. Despite these benefits, LVAD therapy is associated with a number of important adverse events, most notably stroke and device malfunction secondary to device thrombosis. However, as durable LVAD therapy has matured over the past decade, our understanding of patient characteristics and nonpatient factors that contribute to a higher risk of stroke and pump thrombosis has improved, permitting providers greater accuracy in assessing individual patient operative risk and using ris...

Determining Which Prosthetic to Use During Aortic Valve Replacement in Patients Aged Younger than 70 Years: A Systematic Review of the Literature.
CONCLUSIONS: Published literature does not preclude the use of bioprostheses for AVR in younger patients. As new valves are developed, the use of bioprosthetic aortic valves in younger patients will likely continue to expand. Clinical trials are needed to provide surgeons with more accurate guidelines.
