Preventing stroke and systemic embolism in renal patients with atrial fibrillation: focus on anticoagulation.
Abstract Chronic kidney disease and atrial fibrillation (AF) commonly coexist, and data suggest that renal patients have AF rates in excess of double that encountered in the general population. These patients are at increased risk of stroke, regardless of the presence or absence of AF. Furthermore, a lower GFR causes increased thromboembolic risk in patients with AF - independent of other risk factors. Anticoagulation use and the risk of stroke and major bleeding in patients on hemodialysis: From the VIVALDI, a population-based prospective cohort study

Warfarin use, mortality, bleeding and stroke in haemodialysis patients with atrial fibrillation
Conclusions In our population of HD patients with AF, the mortality is very high. OAT is not associated with increased mortality, while antiplatelet drugs are. OAT seems, on the contrary, associated with a better survival; however, it does not decrease the incidence of ischaemic stroke, whereas it increases the incidence of bleeding. Bleeding risk is lower in subjects in whom the INR is kept within the therapeutic range.

Does renal function have incremental predictive value of stroke in atrial fibrillation?
It is well known that both atrial fibrillation (AF) and chronic kidney disease (CKD) are respectively independent predictors of stroke and systemic thromboembolism. A number of epidemiological and observational studies have shown that there are close relationships between AF and CKD [1 –3]. Indeed, the prevalence of AF increases from non-CKD to CKD stages of 1 to 2, 3 and 4 to 5 by 1.0%, 2.8%, 2.7%, and 4.2%, respectively [1], and the prevalence is 7% to 27% in patients with end-stage renal disease who are undergoing hemodialysis therapy [2].
