Cryptogenic Ischemic Stroke in Migraine: Role of Patent Foramen Ovale
CONCLUSION: In a young adult population with CIS, MWA was strongly associated with possibly causal PFO, i.e., with a large shunt or combined with an interatrial septal aneurysm.PMID:35295806 | PMC:PMC8915651 | DOI:10.3389/fpain.2022.823595

Unique Clinicopathologic Subclassifiers of Cryptogenic Cerebral Emboli
CONCLUSIONS: Certain clinical and radiographic features may be useful in predicting the proximal source of occult cerebral emboli, and can be used for cost-effective outpatient diagnostic testing.PMID:35279004 | DOI:10.1016/j.jstrokecerebrovasdis.2022.106427

Cryptogenic stroke and TIA: Suggested diagnostic approach while waiting for evaluation and treatment guidelines
CONCLUSIONS: An individual approach and targeted diagnostics using advanced medical technologies in selected patients, who may benefit from a tailored treatment regimen, can help reveal a probable cause in the majority of strokes and TIAs previously classified as cryptogenic.PMID:35146751 | DOI:10.1111/ane.13590

Orientation of the atrial septum to the inferior vena cava may contribute to the persistent patency of the foramen ovale
Purpose: There is growing evidence that paradoxical embolism through patent foramen ovale (PFO) is a cause for cryptogenic stroke. However, it is still unclear why the foramen ovale fails to close after birth. We studied whether the 3D relations between the atrial septum and the inferior vena cava (IVC) are associated with PFO. Methods: We recruited 30 patients (18-49 years) with a first-ever cryptogenic stroke and 30 age- and sex-matched stroke-free controls. Using cardiac magnetic resonance (CMR), an approach to evaluate the 3D relations between the atrial septum and the IVC was developed. The presence of interatrial ri...

Unexplained arterial thrombosis: approach to diagnosis and treatment
We present a structured approach to diagnosis with consideration of common causes, including atherosclerosis and embolism, as well as uncommon causes, including medications and substances, vascular and anatomic abnormalities, systemic disorders, and thrombophilias. We highlight areas of management that have evolved within the past 5 years, including the use of dual-pathway inhibition in atherosclerotic disease, antithrombotic therapy selection in embolic stroke of undetermined source and left ventricular thrombus, the role of closure of patent foramen ovale for secondary stroke prevention, and the thrombotic potential of c...

Cost-effectiveness analysis of patent foramen ovale closure with Amplatzer plus medical therapy compared to medical therapy in patients with a history of stroke in France
CONCLUSION: Amplatzer plus MT represents a cost-effective treatment option and is associated with lower stroke recurrence compared to MT alone for PFO patients with a prior history of stroke.PMID:34772575 | DOI:10.1016/j.jjcc.2021.10.009

Device-related risk of atrial fibrillation after closure of patent foramen ovale: a systematic review and meta-analysis
ConclusionsThe results suggest lower risk expressed by lower rate of incident AF after PFO closure using Amplatzer PFO Occluder when compared with GORE Occluder. However, these findings are derived from secondary analyses and should be scrutinized using appropriate screening tool for AF following PFO closure in adequately powered randomized clinical trial with a head-to-head design that compares the two devices.

Stroke Prevention in Patients with Patent Foramen Ovale
AbstractPurpose of ReviewPatent foramen ovale (PFO) is widely prevalent and studies have suggested an association with ischemic stroke. In this review, we aim to highlight current management of patients with ischemic stroke in the setting of PFO and discuss some areas of controversy.Recent FindingsUpon reviewing the literature, we have found that the evidence regarding the management of patients with cryptogenic stroke and PFO has come a long way in the past several years, and many uncertainties remain in clinical practice. The Risk of Paradoxical Embolism (RoPE) score helps to predict the probability of a pathogenic PFO, ...
