E-191 Influence of socioeconomic factors on the development of post-stroke depression in aneurysmal subarachnoid hemorrhage survivors
ConclusionThe overall risk of developing a new depression disorder after aSAH was 16% in this retrospective study. The female sex was close to 4 times and substance use were six and half time more likely to develop new depressive symptoms; therefore, these risk factors should be considered when screening patients for depression after aSAH.Disclosures D. Recurrent intracerebral hemorrhage in patients with cerebral amyloid angiopathy: a propensity-matched case –control study
The objective of our study was to compare the readmission rates for recurrent ICH, SAH, and AIS among patients admitted for ICH with and without CAA.MethodsUsing the National Readmissions Database 2016 –2018 we identified patients admitted for ICH with and without a concomitant diagnosis of CAA. Primary outcome of the study was readmission due to ICH. Secondary outcomes included readmissions due to AIS and SAH. Survival analysis was performed, and Kaplan–Meier curves were created to assess for readmissions.ResultsThe study consisted of 194,290 patients with ICH, 8247 with CAA and 186,043 without CAA as a concomitant di...

Long-Term Risk of Ischemic Stroke among Elderly Survivors of Non-Traumatic Subarachnoid Hemorrhage
Conclusions: In a large, heterogeneous national cohort of elderly patients, survivors of SAH had double the long-term risk of ischemic stroke. SAH survivors should be closely monitored and risk stratified for ischemic stroke.Cerebrovasc Dis

Unrecognized Cardiovascular Emergencies Among Medicare Patients
This cohort study estimates the proportions of emergency department visits attributable to symptoms of imminent ruptured abdominal aortic aneurysm, acute myocardial infarction, stroke, aortic dissection, and subarachnoid hemorrhage that end in discharge without diagnosis.

Hospital Discharge Disposition of Stroke Patients in Tennessee.
CONCLUSIONS: Our investigation revealed that the hospital discharge disposition pattern of patients with stroke in Tennessee was associated with the key patient characteristics of selected demographics, clinical indicators, and insurance status. These analyses resulted in the development of an easy-to-use predictive tool for early determination of hospital discharge disposition status. PMID: 28863224 [PubMed - in process]

Characteristics of Accredited Transcranial Doppler Ultrasound Laboratories in the United States
CONCLUSIONSThe results suggest that there are significant differences between IAC applicant laboratories and laboratories represented by Centers for Medicaid and Medicare Services (CMS) claims data. In addition, accurate study reporting, physician training, and ongoing quality improvement activities may not be optimized in laboratories applying for accreditation. With the information learned from this study, educational strategies by professional organizations, including the IAC, can be tailored to help improve TCD practice.

Surgical clipping versus endovascular coiling for elderly patients presenting with subarachnoid hemorrhage
Conclusions In a cohort of Medicare patients, we did not demonstrate a difference in mortality, rate of discharge to rehabilitation, and readmissions between clipping and coiling of ruptured cerebral aneurysms. Clipping was associated with a slightly longer LOS.

Hospital Readmission Rates Among Mechanically Ventilated Patients With Stroke Brief Reports
Conclusions— Approximately one quarter of mechanically ventilated patients with stroke who survive to discharge are readmitted to the hospital within 30 days. Readmission rates are significantly higher in patients with stroke who undergo tracheostomy, but the difference is not clinically meaningful. Thirty-day readmission rates among mechanically ventilated patients with stroke are similar to Medicare beneficiaries hospitalized with major medical diseases such as pneumonia.

Clipping and Coiling of Unruptured Intracranial Aneurysms Among Medicare Beneficiaries, 2000 to 2010 Clinical Sciences
Conclusions— Although outcomes tended to improve over time, increased preventative treatment of unruptured intracranial aneurysms among Medicare beneficiaries did not result in a population-level decrease in SAH rates.

Statins and subarachnoid hemorrhage in Medicare patients with unruptured cerebral aneurysms
ConclusionsStatin use by patients with unruptured cerebral aneurysms was not associated with subarachnoid hemorrhage risk. Given the prior animal experimental studies demonstrating a protective effect, further prospective studies are needed to investigate the potential relationship.

The Case Files: Unusual Headache
By Al-Hashimi, Siddhartha DO; Leavens, John MD A 23-year-old woman with a history of migraine headaches presented to the emergency department for a different-than-usual headache. She had a six-day history of intermittent headaches. The onset was at rest, and there was no history of trauma.   The headache was located behind her left eye, and it radiated into the posterior portion of her head. She characterized it as being 8/10 in intensity. Bright lights were reported as an exacerbating factor. The headache was associated with nausea and multiple episodes of emesis. She had 10 episodes of vomiting the evening prior to arri...
