E-189 Outcomes of tenecteplase versus alteplase after mechanical thrombectomy in a multiethnic stroke consortium registry
ConclusionIn this prospective, multi-center stroke consortium study, IV TNK in comparison to tPA prior to MT for ischemic stroke resulted in non-inferior outcomes related to angiographic reperfusion and functional status at discharge. These findings compliment the current literature and include a large Hispanic US cohort. Further analysis will include expansion of contributing centers and use of propensity scoring.Abstract E-189 Table 1 and 2Disclosures M. Gaub: None. D. Ramaswamy: None. A. Cardentey: None. S. E-152 Neurointervention in the pediatric population: 18-years experience from a single center
ConclusionsAt out institution, multidisciplinary interaction allowed the treatment of a wide variety of cerebrovascular diseases in the pediatric population. Pediatric patients carry high complication risks due to frequent presence of systemic conditions, but joined experience from pediatric neurology, neurosurgery and neurointervention departments allow their treatment with optimal planning and results.Abstract E-152 Table 1Intracranial aneurysms Variable Value Age at diagnosis, mean (SD) or median (IQR) 16 (16–19) Male:Female 12:10 Associated conditions•Moya moya•Connective tissue disorder•Von Wille...

Blood Pressure in Acute Stroke and Secondary Stroke Prevention
This article reviews current guidelines for early management of blood pressure (BP) and highlights the findings of recent investigative works.Recent FindingsIntensive blood pressure reduction after receiving alteplase has not been shown to improve outcomes. Patients with large vessel occlusions may benefit from lower blood pressure targets post-intervention. Retrospective analyses of large intracerebral hemorrhage trials suggest that specific subgroups of patients may disproportionately benefit from or be harmed by intensive BP reduction. Robust data for management of blood pressure in subarachnoid hemorrhage patients is l...

The cost of stroke in a public hospital in Brazil: a one-year prospective study
Conclusions Reperfusion treatments were two-to-four times more expensive than conservative treatment.

Frequency of Hemorrhage on Follow Up Imaging in Stroke Patients Treated With rt-PA Depending on Clinical Course
Conclusions: Frequency of hemorrhagic transformation in Routine follow-up brain imaging and consecutive changes in therapeutic management were different depending on clinical course measured by NHISS score. Introduction Brain imaging 24–36 h after systemic thrombolysis for acute ischemic stroke is recommended in American Stroke Association and European Stroke Organization guidelines (1, 2). Brain imaging is performed to detect secondary bleeding or hemorrhagic transformation in order to adapt medical stroke prevention if necessary. Guideline recommendations are based on the results of the first study on rt-PA...

2016–2017 clinical trials in cerebrovascular neurosurgery
Publication date: Available online 24 October 2018Source: Journal of Clinical NeuroscienceAuthor(s): Wendy Huang, Rose DuAbstractSeveral clinical trials in cerebrovascular neurosurgery were published during 2016–2017. The Collaborative Unruptured Endovascular versus Surgery Trial (CURES) found no difference in outcome between clipping versus coiling of unruptured aneurysms after one year. The Flow Diversion in the Treatment of Intracranial Aneurysm Trial (FIAT) was terminated due to safety concerns. Nimodipine Microparticles to Enhance Recovery While Reducing Toxicity after Subarachnoid Hemorrhage Trial (NEWTON) and othe...

Spontaneous Intraventricular Hemorrhage: When Should Intraventricular tPA Be Considered?
Semin Respir Crit Care Med 2017; 38: 745-759 DOI: 10.1055/s-0037-1607991Spontaneous intracerebral hemorrhage (ICH) is the most common cause of intraventricular hemorrhage (IVH) in adults. Complicating approximately 40% of ICH cases, IVH adds to the morbidity and mortality of this often fatal form of stroke. It is also a severity factor that complicates subarachnoid hemorrhage and traumatic brain injury, along with other less common causes of intracranial bleeding. Medical and surgical interventions to date have focused on limiting ICH and IVH expansion, controlling intracranial pressure, and relieving obstructive hydroceph...

Vessel Wall Enhancement and Blood-Cerebrospinal Fluid Barrier Disruption After Mechanical Thrombectomy in Acute Ischemic Stroke Clinical Sciences
Conclusions—These findings may support the clinical relevance of direct vessel damage and BCSFB disruption after acute stroke and reinforce the need for further improvements in reperfusion strategies. Further validation in larger cohorts of patients is warranted.

Stroke Outcomes and Usage of IV Alteplase during Pregnancy in the Nationwide Inpatient Sample 2005-2010 (P3.100)
ConclusionsStroke remains a serious complication in pregnancy, and mortality is increasing. Usage of tPA for AIS during pregnancy is minimal, with increased mortality compared to all strokes in pregnancy. Research efforts are needed to improve stroke outcomes during pregnancy.Disclosure: Dr. Ouyang has nothing to disclose. Dr. Knopf has nothing to disclose. Dr. Conners has nothing to disclose.
