Transcarotid artery revascularization (TCAR) stenting or angioplasty for intracranial carotid artery stenosis: Case series and novel application
CONCLUSIONS: This highlights a new method for treating intracranial ICA stenosis with a potentially reduced stroke risk. Given the historically higher stroke rates for cervical ICA stenting compared to carotid artery endarterectomy, this method may improve the previously higher stroke rates in endovascular carotid artery treatment, compared to CEA. Although this series is small, it illustrates a novel use for a rising technique that should be further evaluated in a larger study to validate its efficacy as a new treatment modality for surgically inaccessible intracranial disease.PMID:35331034 | DOI:10.1177/15910199221090724

Factors associated with blunt internal carotid artery injury in petrous temporal bone fractures
The incidence of blunt cerebrovascular injury (BCVI), including internal carotid artery (ICA) injury, is approximately 1% of all blunt trauma patients (1-4). Although rare, these injuries can have devastating complications, including stroke, haemorrhage and death, the chance of which may be reduced with early intervention. In light of this, there have been a number of efforts to improve the screening criteria for detection of these injuries in asymptomatic patients (2, 5). It is currently recommended that patients with a petrous temporal bone (PTB) fracture should be considered for BCVI screening with a computed-tomography...

Automated detection and segmentation of intracranial hemorrhage suspect hyperdensities in non-contrast-enhanced CT scans of acute stroke patients
ConclusionThe AI-based algorithm reliably assessed the presence or absence of acute ICHs in this dataset and quantified IPH volumes precisely.Key Points•Artificial intelligence (AI) is able to detect hyperdense volumes on brain CTs reliably.•Sensitivity and specificity are highest for the detection of intraparenchymal hemorrhages.•Interreader reliability for hemorrhage detection shows strong agreement for AI and human readers.

Computed tomography and clinical parameters predict intracerebral hemorrhage expansion
This study aimed to evaluate the association of imaging signs, and to establish a predictive model through selecting highly relevant imaging signs in combination with clinical parameters for hematoma expansion. Intracerebral Hemorrhage (ICH) patients who received 2 consecutive noncontrast computed tomography scans were examined and recruited through January 2014 to December 2020. Demographic information and clinical characteristics were collected. Two experienced radiologists reviewed baseline noncontrast computed tomography images to assess the imaging characteristics. Correlation analysis was analyzed with Pearson and...

Prognostic Factors of the Lethality of Stroke at the Sour ô Sanou University Teaching Hospital of Burkina Faso
Conclusion: The study has identified three prognostic factors of lethality that are the hemorrhagic stroke type, the altered state of consciousness, and the central facial paralysis. Given the high case fatality, it is important to develop and implement effective prevention and management strategies adapted to the resources for the optimal control of stroke in Africa.Cerebrovasc Dis Extra 2022;12:36 –46

Predictors of mortality in acute ischemic stroke treated with endovascular thrombectomy despite successful reperfusion: subgroup analysis of a multicentre randomised clinical trial
Conclusions Symptomatic ICH and high post-thrombectomy NIHSS score are strong predictor of 90-day mortality in acute ischaemic stroke treated with mechanical thrombectomy despite successful reperfusion, as well as high NIHSS score and high glucose level at hospital arrival. However, further studies need to be performed to confirm the association between smoking and mortality.

Advances in computed tomography-based prognostic methods for intracerebral hemorrhage
AbstractSpontaneous intracerebral hemorrhage (ICH) has high morbidity and mortality. Computed tomography (CT) plays an important role in the diagnosis, treatment, and research of cerebrovascular diseases. Non-contrast CT is widely used in the clinical diagnosis of ICH because of its high imaging speed and high sensitivity and specificity in the detection of stroke. Many markers-based CT imaging, quantitative parameters, and artificial intelligence (AI) methods based on CT are increasingly used for the prediction of hematoma expansion (HE), prognosis of ICH, and the evaluation of perihematomal edema (PHE). Therefore, we per...

Intracranial Arterial Calcification and Intracranial Atherosclerosis: Close but Different
CONCLUSION: Our multimodal imaging-based comparison study on intracranial arteriosclerosis demonstrated that intimal IAC, compared with medial IAC, was more often accompanied by the luminal stenosis, larger plaque burden, eccentricity, and IPH, providing strong evidence for clinical evaluation on the mechanism, risk, and prognosis of ischemic stroke.PMID:35211084 | PMC:PMC8861312 | DOI:10.3389/fneur.2022.799429

Head CT for the intensivist: 10 tips and pearls
Minerva Anestesiol. 2022 Feb 24. doi: 10.23736/S0375-9393.22.16200-0. Online ahead of print.ABSTRACTHead imaging is an essential diagnostic tool for the management of patients with most acute neurological emergencies involving the brain. While numerous modalities including magnetic resonance imaging and catheter angiography play a role, computed tomography (CT) of the brain is far and away the most widely utilized technique because of its widespread availability and the fact that it is usually easier to implement in critically ill and potentially unstable patients. CT is particularly useful in identifying acute intracrania...

Outcome of endovascular treatment in acute basilar artery occlusion with National Institutes of Health Stroke Scale score 10-19
CONCLUSIONS: EVT leads to improved outcomes compared to those with SMT alone. Younger age, absence of diabetes mellitus, higher baseline pc-ASPECTSs, and mTICI score of 2b-3 were associated with better functional outcome in the EVT group.PMID:35180699 | DOI:10.3171/2021.12.JNS212543
