Addition of terlipressin to norepinephrine in septic shock and effect of renal perfusion: a pilot study

Benefits of esmolol in adults with sepsis and septic shock: An updated meta-analysis of randomized controlled trials
Conclusions: Esmolol treatment may be safe and effective in decreasing 28-day mortality, controlling heart rate, and providing cardioprotective function, but has no effect on lung injury in patients with sepsis or septic shock after early fluid resuscitation. Improvement in cardiac function may be related to changes in serum inflammatory mediators. No significant adverse effects on tissue perfusion and oxygen utilization were observed.

A Clinical and Physiological Prospective Observational Study on the Management of Pediatric Shock in the Post-Fluid Expansion as Supportive Therapy Trial Era*
CONCLUSIONS: Maintenance-only fluid therapy normalized clinical and myocardial perturbations in shock without compromising cardiac or hemodynamic function whereas fluid-bolus management of WHO shock resulted in high fatality. Troponin and lactate biomarkers of cardiac dysfunction could be promising outcome predictors in pediatric septic shock in resource-limited settings.

Should Hyperoxia Be Avoided During Sepsis? An Experimental Study in Ovine Peritonitis*
Conclusions: In this acute peritonitis model, hyperoxia induced during resuscitation provided better hemodynamics and peripheral microvascular flow and better preserved cerebral metabolism, renal function, and gas exchange. These observations are reassuring with recent concerns about excessive oxygen therapy in acute diseases.

A Rational Approach to Fluid Therapy in Sepsis
(Abstracted from Br J Anaesth, 116:339–349, 2016) The current standard of care for managing patients with severe sepsis and septic shock is aggressive fluid resuscitation, until a central venous pressure of greater than 8 mm Hg is achieved. The rationale behind this approach is to increase a patient’s stroke volume, which in turn improves cardiac output and organ perfusion.

Time course of cytokines, hemodynamic and metabolic parameters during hyperthermic intraperitoneal chemotherapy.

Arginine infusion in septic shock patients increases nitric oxide production without hemodynamic instability
In conclusion, a4-fold increase of plasma arginine with intravenous arginine infusion in sepsis stimulates de novo arginine and NO production and reduces whole body protein breakdown. These potential beneficial metabolic effects occurred without negative alterations in hemodynamic parameters, although improvement of regional perfusion could not be demonstrated in the eight patients with septic shock that were studied.
