The Effect of Dialysis Shift on Depression and Sleep Disturbances in Patients on Chronic Hemodialysis
Nephrol Nurs J. 2023 Jul-Aug;50(4):293-303.ABSTRACTStudies suggest that the hemodialysis (HD) shift on which a patient dialyzes impacts sleep disorders, depressive symptoms, quality of life, and mortality. However, study findings have been limited and inconsistent. The purpose of this project was to determine the relationship between HD shifts (morning, afternoon, and evening), and depression and sleep quality disturbance. Forty-one participants were recruited to participate in this project. Participants on the third shift had the highest levels of depression (M = 11.67) and poorest sleep quality (M = 14.5), compared to th...

Assessing the Impact of Intra-Dialytic Exercise Regimen on Depression
Nephrol Nurs J. 2023 Jul-Aug;50(4):289-303.ABSTRACTDepression is a common mental health concern among patients with chronic kidney disease. This population has a higher prevalence of hospitalization than those without depression. Exercising during dialysis, specifically intra dialytic pedal cycling, as an intervention can improve patients' overall well-being and promote a better quality of life both mentally and physically.PMID:37695514

Encapsulation of Dicranopteris linearis extract using cellulose microparticles for antiulcer medication
Int J Biol Macromol. 2023 Sep 7:126795. doi: 10.1016/j.ijbiomac.2023.126795. Online ahead of print.ABSTRACTDicranopteris linearis (DL) is a fern in the Gleicheniaceae family, locally known as resam by the Malay community. It has numerous pharmacological benefits, with antiulcer and gastroprotective properties. Peptic ulcer is a chronic and recurring disease that significantly impacts morbidity and mortality, affecting nearly 20 % of the world's population. Despite the effectiveness of peptic ulcer drugs, there is no perfect treatment for the ailment. Encapsulation is an advanced technique that can treat peptic ulcers by in...

Nephroscope-assisted single trocar peritoneal dialysis catheter insertion improves catheter survival compared with laparoscopic technique: Ten-year experience of a single center
CONCLUSIONS: Compared with the traditional laparoscopic approach, the nephroscopic technique effectively prolonged catheter survival and reduces health care cost by reducing infectious complications. The low complication rate during the learning phase of surgery makes the procedure safe for patients and surgeons.PMID:37689515 | DOI:10.1016/j.asjsur.2023.08.143

The impact of the early COVID-19 pandemic on healthcare system resource use and costs in two provinces in Canada: An interrupted time series analysis
CONCLUSIONS: Reduced utilization of healthcare services in the overall population outweighed utilization by COVID-19 patients in 2020. Meeting the needs of all patients across all services is essential to maintain resilient healthcare systems.PMID:37682823 | DOI:10.1371/journal.pone.0290646

Electrolyte imbalance causes suppression of NK and T cell effector function in malignant ascites
CONCLUSION: Our data suggest a novel electrolyte-based mechanism of immunosuppression in malignant ascites of patients with peritoneal carcinomatosis. We show for the first time that the immunosuppression of NK cytotoxicity in coculture assays is correlated to patient poor survival. Therapeutic application of sodium channel inhibitors may provide new means for restoring immune cell activity in ascites or similar electrolyte imbalanced environments.PMID:37684704 | DOI:10.1186/s13046-023-02798-8

Dialysis after kidney transplant failure: how to deal with this daunting task?
J Nephrol. 2023 Sep 7. doi: 10.1007/s40620-023-01758-x. Online ahead of print.ABSTRACTThe best treatment for patients with end-stage kidney disease is kidney transplantation, which, if successful provides both a reduction in mortality and a better quality of life compared to dialysis. Although there has been significant improvement in short-term outcomes after kidney transplantation, long-term graft survival still remains insufficient. As a result, there has been an increase in the number of individuals who need dialysis again after kidney transplant failure, and increasingly contribute to kidney transplant waiting lists. ...

Obesity, Weight Gain, and Fluid Overload in Peritoneal Dialysis
Front Nephrol. 2022 Jun 28;2:880097. doi: 10.3389/fneph.2022.880097. eCollection 2022.ABSTRACTObesity is a global epidemic that has a complicated pathogenesis as well as impact on the outcome of peritoneal dialysis (PD) patients. In this review, the prevalence of obesity in incident PD patients as well as the phenomenon of new-onset glucose intolerance after PD will be reviewed. Published literature on the effect of obesity on the survival and incidence of cardiovascular disease in PD patients will be discussed. Particular emphasis would be put on literature that compared the impact of obesity on the outcome of hemodialysi...

Sodium First Approach, to Reset Our Mind for Improving Management of Sodium, Water, Volume and Pressure in Hemodialysis Patients, and to Reduce Cardiovascular Burden and Improve Outcomes
Front Nephrol. 2022 Jul 7;2:935388. doi: 10.3389/fneph.2022.935388. eCollection 2022.ABSTRACTNew physiologic findings related to sodium homeostasis and pathophysiologic associations require a new vision for sodium, fluid and blood pressure management in dialysis-dependent chronic kidney disease patients. The traditional dry weight probing approach that has prevailed for many years must be reviewed in light of these findings and enriched by availability of new tools for monitoring and handling sodium and water imbalances. A comprehensive and integrated approach is needed to improve further cardiac health in hemodialysis (HD...

System-Level Strategies to Improve Home Dialysis: Policy Levers and Quality Initiatives
Clin J Am Soc Nephrol. 2023 Sep 6. doi: 10.2215/CJN.0000000000000299. Online ahead of print.ABSTRACTAdvocacy and policy change are powerful levers to improve quality of care and better support patients on home dialysis. While the kidney community increasingly recognizes the value of home dialysis as an option for patients who prioritize independence and flexibility, only a minority of patients dialyze at home in the United States. Complex system-level factors have restricted further growth in home dialysis modalities, including limited infrastructure, insufficient staff for patient education and training, patient-specific ...
