Treatment of digital ulcers in systemic sclerosis: recent developments and future perspectives
AbstractDigital ulcers (DUs) comprise the main manifestation of vasculopathy and are a major cause of disability in patients with systemic sclerosis (SSc). A literature search in Web of Science, PubMed and Directory of Open Access Journals was performed in December 2022 to identify articles published in the last decade regarding the management of DUs. Prostacyclin analogues, endothelin antagonists and phosphodiesterase 5 inhibitors have shown promising results both as a stand-alone treatment and in combination for the treatment of existing and prevention of new DUs. Moreover, autologous fat grafting and botulinum toxin inj...

Continuous Tunneled Infraclavicular Nerve Catheter for Palliative Management of Critical Limb Ischemia: A Case Report
We describe the successful use of a continuous local anesthetic infusion via an infraclavicular nerve catheter to control severe refractory ischemic upper limb pain in a patient with metastatic lung cancer for whom surgical and pharmacological intervention was unsuccessful. As her opioid requirements increased due to worsening ischemic pain, she subsequently developed opioid toxicity, hence prompting the palliative use of a tunneled infraclavicular nerve catheter under ultrasound guidance to minimize opioid requirements. Her opioid requirements tailed down subsequently with the successful insertion of the infraclavicular n...

Efficacy of Massage Techniques for Hypertrophic Burn Scars - A Systematic Review of Literature
J Burn Care Res. 2023 Sep 12:irad140. doi: 10.1093/jbcr/irad140. Online ahead of print.ABSTRACTDespite advancements in burn care, evidence estimates that pathological scarring occurs in 32% to 75% of cutaneous burns. Scar massage therapy is an under researched method of management for hypertrophic burn scars which has scope to be a low-cost treatment alternative. The aim of this systematic review was to determine the efficacy of scar massage techniques for common hypertrophic burn scar symptoms such as contraction, pruritus, pain and visibility. The keywords and corresponding MeSH terms were inputed into PubMed, EMBASE, Co...

Quality of life, fatigue, sleep quality, and mental health in systemic lupus erythematosus patients with a high cardiovascular risk profile
Lupus. 2023 Sep 12:9612033231202582. doi: 10.1177/09612033231202582. Online ahead of print.ABSTRACTSystemic lupus erythematosus (SLE) patients report worse health-related quality of life (HRQL), fatigue, anxiety, depression, and sleep quality, when compared to the general population and other chronic diseases. Furthermore, cardiometabolic diseases are highly prevalent in SLE and are also associated with these parameters. Thus, it is plausible to suggest that SLE patients with a high cardiovascular risk may report worse results for these parameters. The aim of the study is to describe HRQL, fatigue, anxiety and depression s...

6 Things to Do if Your Doctor Isn ’ t Listening to You
Liz Helms still remembers the way her doctor looked at her 30 years ago, when she described the severe pain she was experiencing in the area around her jaw. She told him she had limited facial movement and frequent muscle spasms, and that it felt like she was being struck by lightning again and again. She cataloged the ways her life had turned upside down. “But I could see it in his face—he either wasn’t listening or didn’t believe me,” Helms says. She remembers thinking: “They treat animals better than they’re treating humans.” [time-brightcove not-tgx=”true”] ...

Functional Alterations of the Basal Ganglia Are Associated with Voluntary Activation of the Core Stabilizing Muscles in Patients with Chronic Low Back Pain: A Cross-Sectional Study
CONCLUSIONS: Grey matter alterations may be confined to regions responsible for perception, motor control, and emotion regulation in patients with CLBP. The interrupted EC from the basal ganglia to the default mode network might be involved in the impairment of voluntary activation of the core stabilizing muscles.PMID:37693681 | PMC:PMC10484657 | DOI:10.1155/2023/2028379

Effect of Oral Prednisolone on Pain after Tonsillectomy with Sutures: A Randomized Clinical Trial
CONCLUSION: Numerous studies have been conducted on the effect of intravenous corticosteroids on this pain. However, there is no consensus on the analgesic role of oral corticosteroids for post-tonsillectomy pain. The present study showed that oral prednisolone is effective on post-operative pain compared to a placebo.PMID:37694239 | PMC:PMC10492605 | DOI:10.4103/abr.abr_410_22

Identifying populations with chronic pain in primary care: developing an algorithm and logic rules applied to coded primary care diagnostic and medication data
CONCLUSIONS: Estimates of chronic pain prevalence using structured healthcare data have previously shown lower prevalence estimates for chronic pain than reported in community surveys. This has limited the ability of researchers and clinicians to fully understand and address the complex multifactorial nature of chronic pain. Our study demonstrates that it may be possible to establish more representative prevalence estimates using structured data than previously possible. Use of logic rules offers the potential to move systematic identification and population-based management of chronic pain into mainstream clinical practic...

An Unusual Cause of Fever, Neck Pain, and Neck Stiffness: Acute Calcific Tendinitis of the Longus Colli Muscle
J Emerg Med. 2023 Jun 10:S0736-4679(23)00316-5. doi: 10.1016/j.jemermed.2023.05.023. Online ahead of print.ABSTRACTBACKGROUND: Acute calcific tendinitis (ACT) of the longus colli muscle (LCM) is an inflammatory response due to deposition of calcium hydroxyapatite crystals. It is typically correlated with whiplash and overuse injuries. A common presentation of this inflammatory response is acute but progressive neck pain. It is a rare but important cause of neck pain that should be considered on a differential diagnosis when distinguishing between life-threatening conditions and non-life-threatening causes of neck pain.CASE...
