The Efficacy of Stellate Ganglion Blockade in Herpetic Neuralgia: A Case Report
Cureus. 2023 Aug 20;15(8):e43805. doi: 10.7759/cureus.43805. eCollection 2023 Aug.ABSTRACTThe varicella-zoster virus can reactivate in patients with impaired cell-mediated immunity, resulting in herpes zoster (HZ) and sometimes in herpetic neuralgia (HN). Stellate ganglion blockade (SGB) has been studied for the treatment of HN and postherpetic neuralgia (PHN), but its effectiveness in preventing PHN in patients with acute HZ is not well-established. Here, we present a case of a 75-year-old woman who underwent SGB for HN of the right upper limb. The patient had been treated with antivirals and various analgesics, but adver...

Clinical and Epidemiological Profile of Herpes Zoster and Its Complications in a Tertiary Care Center of Bihar: A Prospective Study
Conclusion A broader understanding of the clinical and epidemiological factors of herpes zoster and post-herpetic complications is important as this disease constitutes a considerable burden in a tertiary care center and if not treated adequately, the after-effects might last for many years altogether. Hence, early diagnosis and initiation of adequate antiviral therapy as well as pain management is the key aspect of management.PMID:37719532 | PMC:PMC10503883 | DOI:10.7759/cureus.43560

Herpetic esophagitis in healthy young adult
Clin J Gastroenterol. 2023 Sep 15. doi: 10.1007/s12328-023-01852-4. Online ahead of print.ABSTRACTHerpetic esophagitis (HE) is common in immunocompromised patients but rarely reported in healthy young adults. A 28-year-old healthy male patient visited our clinic with a chief complaint of chest pain. An esophagogastroduodenoscopy revealed widely spread superficial multiple ulcers in the middle to the distal esophagus, and repeated biopsies from the ulcer were performed but revealed inconsistent findings with HE. Thus, he was clinically diagnosed with HE based on endoscopic findings and serologic tests of immunoglobulin M (I...

Varicella Zoster Virus-Specific Hyperimmunoglobulin in the Adjuvant Treatment of Immunocompromised Herpes Zoster Patients: A Case Series
CONCLUSION: This case series demonstrates highly satisfactory treatment effectiveness and tolerability for VZV-IgG in the adjuvant treatment of immunocompromised HZ patients and supports early intensification of HZ therapy in patients at high risk of severe disease progression.PMID:37704912 | DOI:10.1007/s13555-023-01019-6

Postherpetic Trigeminal Neuralgia of the V2 Branch Treated with Electrodes Placed through the Foramen Ovale: A Case Report
In this report, we present a case of post-herpetic trigeminal neuralgia of the V2 branch, exhibiting findings of unusual involvement of the trigeminal nerve. Notably, the patient was treated using electrodes placed through the foramen ovale. PAQR8 and RAQR9 expression is altered in the ventral tegmental area of aged rats infected with varicella zoster virus
Mol Pain. 2023 Sep 12:17448069231202598. doi: 10.1177/17448069231202598. Online ahead of print.ABSTRACTInfection with varicella zoster virus (VZV) results in chicken pox and reactivation of VZV results in herpes zoster (HZ) or what is often referred to as shingles. Patients with HZ experience decreased motivation and increased emotional distress consistent with functions of the ventral tegmental area (VTA) of the brain. In addition, activity within the ventral tegmental area is altered in patients with HZ. HZ primarily affects individuals that are older and the VTA changes with age. To begin to determine if the VTA has a r...

Metabolomics profiling in predicting of post-herpetic neuralgia induced by varicella zoster
Sci Rep. 2023 Sep 11;13(1):14940. doi: 10.1038/s41598-023-42363-z.ABSTRACTTo explore potential metabolomics biomarkers in predicting post-herpetic neuralgia (PHN) induced by herpes zoster (HZ). A total of 90 eligible patients were prospectively enrolled and assigned into an acute pain (ACP) group and a PHN group. Serum samples were collected before clinical intervention to perform metabolomics profiling analyses using gas chromatography mass spectrometry (GC-MS). Key metabolites were identified using partial least squares discriminant analysis (PLS-DA). A binary logistic regression was used to build a combined biomarker mo...

Unilateral acute iris transillumination syndrome with glaucoma and iris pigment epithelium dispersion simulating iris melanoma
CONCLUSION AND IMPORTANCE: Unilateral acute iris transillumination syndrome with diffuse iris pigment epithelial loss can simulate iris melanoma. Prompt herpes viral studies may be informative.PMID:37680309 | PMC:PMC10481171 | DOI:10.1016/j.ajoc.2023.101912

Comparison of the safety and efficacy of radiofrequency thermocoagulation with percutaneous balloon compression for treating trigeminal neuralgia: a systematic review and meta-analysis
ConclusionIn the treatment of TN, both RFT and PBC can achieve good short-term efficacy, and no significant differences were noted between the outcomes of the two approaches. Compared with RFT, PBC may result in a lower pain score and recurrence rate in the medium and long terms, but it is a higher incidence of cold sores, and the decrease of masticatory muscle strength is more obvious.
