Bone reconstruction with vascularized free fibula in the treatment of osteomyelitis of the distal radius: a case series
Abstract Osteomyelitis of the distal radius and wrist with bone defect presents a reconstructive challenge. Free fibula grafts have been successfully used in post tumor resection reconstruction of the wrist. We applied these techniques to the reconstruction of wrist adjacent osteomyelitis associated bone defects and report on the feasibility of the technique in this context. We retrospectively evaluated 4 patients treated from January 2009 to February 2017 with a free fibular osteocutaneous flap for distal radius osteomyelitis. Previous operations, defect size, time of operation, type of fixation, complete bone union, an...

A Very Unusual Presentation of Miliary Tuberculosis and Osteomyelitis as an Incidental Finding of Musculoskeletal Pain
Cureus. 2023 Aug 27;15(8):e44207. doi: 10.7759/cureus.44207. eCollection 2023 Aug.ABSTRACTTuberculosis (TB) is a highly infectious disease that takes the primary or latent route. In primary TB, the patient often presents with constitutional symptoms such as cough, fever, weight loss, and hemoptysis. This 83-year-old patient was sent to the emergency department (ED) by her primary care physician after abnormal imaging for acute back and neck pain. Imaging revealed pulmonary TB with possible tuberculosis osteomyelitis versus metastatic carcinoma to the cervical vertebrae. The case's unique presentation gives light to the nee...

Vertebral osteomyelitis due to < em > Lactobacillus paracasei < /em > in a diabetic patient. A case report and literature review
We describe a rare case of spondylodiscitis due to Lactobacillus paracasei. A 74-year-old diabetic male was evaluated for fever and back pain. Blood cultures and vertebral biopsy were positive for Lactobacillus paracasei. He often took laxatives and probiotics for chronic constipation. After target treatment the patient improved but he died for a heart attack two months after the end of the treatment. Although Lactobacillus paracasei is usually not pathogenic, sepsis is described in immunocompromised patients while vertebral osteomyelitis is rare.PMID:37701384 | PMC:PMC10495059 | DOI:10.53854/liim-3103-13

Cardiac Tamponade due to Purulent Pericarditis
A 65-year-old man was admitted to the hospital because of lower back pain that had persisted for 1 month. Pyogenic vertebral osteomyelitis was suspected based on elevated C-reactive protein (CRP) levels and CT findings of a compression fracture of the 12th thoracic vertebra. Empirical antimicrobial therapy was initiated after blood cultures were obtained. On the third day of treatment, his blood pressure rapidly decreased and respiratory failure developed, necessitating transfer to the intensive care unit.

Risk factors of deep vein thrombosis in children with osteomyelitis
CONCLUSIONS: Admission to ICU, sepsis, higher D-dimer, higher body temperature during hospitalization, and MRSA are risk factors for thrombosis. MRSA is the independent risk factor for DVT. For patients with related risk factors, timely ultrasound examination of the infected site should be considered to achieve early detection and treatment.PMID:37624766 | DOI:10.1080/07853890.2023.2249011

Complex Case of Skull Base Osteomyelitis Secondary to Chronic Otitis Media
We present a case of a 58-year-old male with type II diabetes managed with metformin and insulin, who presented to the clinic with left chronic otitis media, persistent drainage, a stenotic meatus, and a prior history of 3 canal wall-down mastoidectomies and antibiotic therapy. A revision tympanoplasty with mastoidectomy was performed, and during the postoperative period, the patient had persistent pain and otorrhea, which were managed with opioids and several courses of antibiotic therapy. After symptoms persisted, imaging and culture ultimately led to the diagnosis of fungal skull base osteomyelitis, which was eventually...
