Opioid-free percutaneous nephrolithotomy: an initial experience
ConclusionOur results show that there were no significant differences in pain-related encounters between those who received ketorolac and oxycodone –acetaminophen following PCNL. A non-opioid pathway may mitigate the potential risk associated with opioid prescription without compromising analgesia. Prospective comparative studies are warranted to confirm feasibility.

The effect of morphine on rat microglial phagocytic activity: An in vitro study of brain region-, plating density-, sex-, morphine concentration-, and receptor-dependency
Opioids have long been used for clinical pain management, but also have addictive properties that have contributed to the ongoing opioid epidemic. While opioid activation of opioid receptors is well known to contribute to reward and reinforcement, data now also suggest that opioid activation of immune signaling via toll-like receptor 4 (TLR4) may also play a role in addiction-like processes. TLR4 expression is enriched in immune cells, and in the nervous system is primarily expressed in microglia.

Evolution of Perioperative Pain Management in Shoulder Arthroplasty
Orthop Clin North Am. 2023 Oct;54(4):435-451. doi: 10.1016/j.ocl.2023.04.004. Epub 2023 Jun 20.ABSTRACTHistorically, opioids have been used as a primary conservative treatment for pain related to glenohumeral osteoarthritis (GHOA). However, this practice is concerning as it often leads to overuse, which has contributed to the current epidemic of addiction and overdoses in the United States. Studies have shown that preoperative opioid use is associated with higher complication rates and worse outcomes following surgery, particularly for shoulder arthroplasty. To address these concerns, perioperative pain management for shou...

Post-Procedure Analgesic Management
This article will go in depth to explain the analgesic tools practitioners can implement for an effective low-risk protocol, including a combination of NSAIDS and acetaminophen approach, using long-acting local anesthetics such as Exparel, pregabalin, gabapentin, ketamine, dexmedetomidine, and corticosteroids, and enhanced recovery after surgery protocols.

Increasing Provider Self-Efficacy to Manage Chronic Cancer Pain
The United States is currently amid a significant opioid epidemic (Jackson,  2019). In the 1990s, pain was marked as the fifth vital sign and providers were required to effectively treat pain with opioid therapy (Jones et al., 2018). Frequent use of opioids has been associated with opioid tolerance, misuse, and opioid associated mortality (Jones et al., 2018). In 2016 alone there were approximately 42,000 opioid associated-deaths (Jackson, 2019; Jones et al., 2018).

The different cell-specific mechanisms of voluntary exercise and forced exercise in the nucleus accumbens
Neuropharmacology. 2023 Sep 8:109714. doi: 10.1016/j.neuropharm.2023.109714. Online ahead of print.ABSTRACTPhysical inactivity is a global epidemic. People who take the initiative to exercise will feel pleasure during the exercise process and stick with it for a long time, while people who passively ask for exercise will feel pain and cannot stick with it. However, the neural mechanisms underlying voluntary and forced exercise remain unclear. Here, we report that voluntary running increased the frequency and amplitude of miniature excitatory postsynaptic currents (mEPSC) but decreased membrane excitability in D1R-MSNs, whe...

Framing Cancer Survivors' Access to and Use and Disposal of Prescribed Opioids Within the Opioid Epidemic
Oncol Nurs Forum. 2022 Dec 16;50(1):25-34. doi: 10.1188/23.ONF.25-34.ABSTRACTPURPOSE: To explore cancer survivors' access to and use and disposal of opioids in the context of the opioid epidemic.PARTICIPANTS & SETTING: Community-based recruitment strategies were employed for individuals aged 18 years or older who were previously diagnosed with cancer, completed cancer treatment within the past five years, or were cancer free, and who were prescribed opioids for cancer-related pain.METHODOLOGIC APPROACH: This qualitative study used semistructured interviews. Data were analyzed using applied thematic analysis techniques....
