Physician Palliative Education Associated With High Use of Hospice Care Service
CONCLUSIONS: Physicians' palliative education was found to be an independent factor associated with higher use of hospice care. The findings suggest increasing palliative and hospice education among physicians so that they can ensure that their patients have high-quality end-of-life medical care in an aging society.PMID:33960206 | DOI:10.1177/10499091211014160

Time for a new Paradigm
Roberto Savio is founder of IPS Inter Press Service and President EmeritusBy Roberto SavioROME, Jan 8 2019 (IPS)The person most qualified to write the foreword for this latest work by Riccardo Petrella, In the Name of Humanity, would actually be Pope Francis, who, using other words but speaking of values and making denouncements, has often argued what the reader will find in the following pages. I quote him, because words like “solidarity”, “equality”, “social justice” or “participation” – now used only by Pope Francis I – have now disappeared from today’s political...

Olympus rejiggers medical biz
Olympus (TYO:7733) last week announced its plan to rejigger its medical device business with an eye toward gaining share in the U.S. market. The Japanese conglomerate weathered an accounting scandal in 2011 after firing its first foreign CEO, Michael Woodford, after he blew the whistle on unexplained payments worth around $1.7 billion and demanded the resignation of the former chairman and vice president. In September 2012 the company and 3 former executives pleaded guilty in Japan to cover-up charges. By 2014 Olympus was caught up in an FDA warning about power morcellators, which were found to pose a cancer risk;...

Unarousable child with a short bowel
Unarousable child with short bowel A 4-year-old boy was admitted with progressive lethargy of a few hours’ duration and no other symptoms. His medical history was relevant for short bowel syndrome (SBS), following neonatal volvulus, with residual bowel length of 23 cm and intact ileocecal valve. He had similar self-limiting episodes in the past, after weaning parenteral nutrition, especially after eating large meals. The day before, he had consumed a large amount of apples. Arterial blood gas (ABG) analysis showed metabolic acidosis with normal lactacidaemia (pH 7.09, pCO2 19 mm Hg, pO2 101 mm Hg, HCO3 5.8 mmol/L, BE...

Medical Podcasting in Low- and Middle-Income Countries: A needs Assessment and Vision for the Future
Teach Learn Med. 2021 Feb 15:1-7. doi: 10.1080/10401334.2021.1875834. Online ahead of print.ABSTRACTPhenomenon: Physician shortages in low- and middle-income countries (LMIC) have led to increased interest in using e-learning tools for training. Organic digital education (ODE)-digital scholarship largely created outside of formal medical curricula-has increased in popularity over the past decade. Medical podcasting has become one of the most prominent asynchronous ODE sources for learners in high-income (HI) countries; there have been no previous attempts to characterize their use in LMIC. Approach: Listener data from a 2-...

Franz Joseph Gall on the Cerebellum as the Organ for the Reproductive Drive
This article examines Gall’s earlier subcortical organs, with an emphasis on why he associated the cerebellum with this drive. It draws from accounts by several physicians, who attended his Vienna lectures or heard him speak in Germany and the Netherlands in 1805–1806 [i.e., before he published his finalized list in his Anatomie et Physiologie (1810–1819)]. These early accounts show that early on he localized at least four faculties in brainstem structures, including a reproductive drive in the cerebellar cortex. He based his structure–function association primarily on cranial differences be...

Organization and Outcomes of Integrated Inpatient Medical and Psychiatric Care Units: A Systematic Review
CONCLUSIONS: IMPUs may help shorten LOS. IMPUs should focus care on patients with complex conditions and high acuity to maximize health system value. Implementing compulsory admission facilities; qualified psychiatric, medical, and nursing staff involvement; and cross-disciplinary training may improve IMPUs' capacity to treat high-acuity patients. Future research should relate IMPU designs to intended outcomes.PMID:34407632 | DOI:10.1176/appi.ps.202000416

What Do You Do For Breast Milk Jaundice?
Discussion Almost all infants in the first few days of life have some elevation of their bilirubin because of the various physiologic changes they undergo in the transition to extra-uterine life. This is a common problem managed by many different inpatient and outpatient healthcare providers. This hyperbilirubinemia is usually unconjugated and resolves in the first 1-2 weeks of life. However, when it does not resolve, health care providers have 3 general paths to follow when considering a cause: inadequate breast milk intake, an underlying organic cause or breast milk jaundice. Inadequate breastfeeding is not uncommon and...

What Are Common Presentations for Factitious Disorder?
Discussion Factitious disorder (FD) is when patients fabricate illness in themselves or another person without obvious gain. FD has had other terminology and in the past has been called Munchausen Syndrome (if FD in themselves) or Munchausen Syndrome by proxy or FD by proxy (if FD being perpetuated in another person). The current DSM-V terminology is FD imposed on self or FD imposed on another. Malingering (sometimes referred to as simulation) is similar but those that fabricate illness or medical need have an obvious external reward such as gaining access to public social service benefits, free hospital room and board, or...
