Conclusions and Clinical RelevanceDiagnostic testing is not harmonised, with marked variability in the NMBA panels used to identify safe alternatives. Chlorhexidine and latex are not part of routine testing in many centres.Poor access to services and patient information provision require attention. Local anesthetics allergy: who should be tested?
Conclusion. The most common referral reason for testing with LA was a history of DHR to drugs other than LAs, whereas asthma was the third most common referral reason. Patients with a history of multiple DHR may be considered for testing with LAs. Asthmatics and those with other allergic diseases without a history of drug / LA allergy do not need to be tested with LA. PMID: 29384109 [PubMed - as supplied by publisher]

Predictive value of allergy tests for neuromuscular blocking agents: tackling an unmet need
ConclusionSkin test and BAT have an excellent negative predictive value in our series. The uneventful re‐exposure of rocuronium in patients with an isolated positive sIgE result to rocuronium calls into question the predictive value of this assay and suggests sIgE serology to be less clinically predictive than the functional investigations relying upon activation of mast cells or basophils. Global patterns of drug allergy-induced fatalities: a wake-up call to prevent avoidable deaths
Purpose of review To identify patterns and key issues though a systematic review in order to support prevention strategies and reduce avoidable deaths related to drug-induced anaphylaxis (DAF). Recent findings DAF rate has been estimated by 0.13–0.53/106 population/year. General global trends of DAF are increasing over time, mostly occurring at healthcare settings (62%) with a similar gender distribution and an average age of 53 years. Risk of Immediate-Type Allergy to Local Anesthetics is Overestimated —Results from 5 Years of Provocation Testing in a Danish Allergy Clinic
Conclusions None of the 164 patients with suspected immediate-type allergy to LAs reacted on provocation. Thus, no patients have been diagnosed with an immediate allergy to LAs in our regional allergy clinic in the 5-year period studied, and allergy to LAs must be considered very rare. Alternative mechanisms should be considered, but if symptoms are consistent with allergy, other potential allergens should be investigated.

The Importance of Prolonged Provocation in Drug Allergy — Results From a Danish Allergy Clinic
Conclusions Only 11.0% of the provocations were positive. Importantly, only 1 of 5 patients tested positive on the first dose, indicating that prolonged exposure should always be considered when drug provocation is included in allergy investigations. Most provocations were with penicillins, reflecting the pattern of antibiotic use in Denmark, which differs from that in other countries, especially outside Northern Europe.
