Hyperimmunoglobulinemia E and efficacy of elimination diet in two patients with Schnitzler syndrome.
CONCLUSION: The high serum levels of total IgE may be associated with chronic urticaria activity, severe disease course and a poor response to treatment with antihistamines, and may be considered a possible marker of a subset of patients with SS showing a good response to the restriction diet. In general, we can assume that elimination diet can have an influence on the skin lesions and other symptoms of SS as well as on total IgE and IgM levels, but such association, the underlying mechanisms and the reasons for excessive IgE synthesis should be investigated in further studies. Immediate hypersensitivity reactions to corticosteroids.
CONCLUSION: Corticosteroids seem to be rare causes of immediate hypersensitivity reactions but possibly are misdiagnosed or under-reported relative to their worldwide use. Physicians should be cognizant of this entity and identify safe alternative preparations. PMID: 26211812 [PubMed - as supplied by publisher]

Pure red cell aplasia as a presenting feature in systemic lupus erythematosus and association with thymoma, hypothyroidism and hypoparathyroidism: a case report and literature review.
Abstract This paper presents a 54-year-old female with lupus whom severe anaemia due to pure red cell aplasia (PRCA) was the first manifestation. There was seven years interval between PRCA onset and diagnosis of lupus. Thymectomy due to thymoma had been carried out six years before but anaemia sustained. Hypothyroidism and hypoparathyroidism were other associated diseases. Severe anaemia and the need for monthly blood infusions were resolved following treatment with Prednisolone, Hydroxychloroquine and Levothyroxine. PMID: 24338260 [PubMed - in process]

The Effect of Conventional Immunosuppressive Therapy on Cytokine Serum levels in Pemphigus Vulgaris Patients.
In conclusion, IL-1β and IL-6 could be involved in the pathogenesis of pemphigus vulgaris. The positive trend of IL-22 is a new finding and should be confirmed by further studies. PMID: 24659121 [PubMed - in process]

Skin testing for immediate hypersensitivity to corticosteroids – a case series and literature review
ConclusionsSkin testing can provide sufficient evidence to diagnose allergy in patients with a clear history of immediate hypersensitivity to corticosteroids such as anaphylaxis. Both skin prick and intradermal tests should be used. There is evidence of cross‐reactivity between steroids so a panel is recommended. False positive and false negative reactions do occur, however the frequency is unknown. Challenge remains the only definitive way to demonstrate a safe alternative to use. This article is protected by copyright. All rights reserved.

A case of sulfasalazine-induced hypersensitivity syndrome confirmed by enzyme-linked immunospot assay.
Authors: Phatharacharukul P, Klaewsongkram J Abstract A 24-year-old male with a history of spondyloarthropathy presented with high fever, cervical lymphadenopathy and generalized maculopapular rash. He was treated with prednisolone for chronic uveitis before being switched to sulfasalazine 3 weeks prior to admission. Laboratory findings revealed marked leukocytosis with frequent atypical lymphocytes. Sulfasalazine was discontinued and the etiology of mononucleosis syndrome explored. During admission, he developed acalculous cholecystitis and hypotension. All symptoms quickly improved following administration of sys...

Eosinophilic Granulomatosis with Polyangiitis preceding allergic bronchopulmonary aspergillosis.
Authors: Lee W, Teo FS, Santosa A, Teng GG Abstract A 61-year-old Chinese man with long-standing, stable Eosinophilic Granulomatosis with Polyangiitis (EGPA) and asthma, presented with acute hypoxemia and declining obstructive pulmonary function. Elevated serum IgE levels, positive Aspergillus fumigatus specific IgE and CT findings of central bronchiectasis with small airway mucoid impaction confirmed new development of Allergic Bronchopulmonary Aspergillosis (ABPA). The maintenance therapy for EGPA, azathioprine, was discontinued. Prednisolone 0.5mg/kg/day and Itraconazole improved his symptoms and IgE levels. To ...

Slow desensitization of imatinib-induced nonimmediate reactions and dynamic changes of drug-specific CD4(+)CD25(+)CD134(+) lymphocytes.
CONCLUSION: Slow desensitization is a helpful procedure in treating patients with imatinib-induced nonimmediate reactions other than simple maculopapular exanthema. The reduced percentages of imatinib-induced CD4(+)CD25(+)CD134(+) T cells in these patients may be associated with immune tolerance. PMID: 27788881 [PubMed - in process]

High-dose nebulized budesonide is effective for mild asthma exacerbations in children under 3 years of age.
Conclusion. For children < 3 years old with mild asthma exacerbations, high-dose nebulized budesonide therapy is equally as effective as systemic steroid therapy. PMID: 28120603 [PubMed - in process]

Phenotyping patients with chronic cough: Evaluating the ability to predict the response to anti-inflammatory therapy.
CONCLUSION: These results suggest that prior investigation may not predict response to anti-inflammatory treatment, which may be consequent on localized leukotriene-mediated inflammation. TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT02479074. PMID: 29508715 [PubMed - in process]

Multiple Evanescent White Dot Syndrome: A Case Report and Experience with Corticosteroid Therapy.
Abstract Multiple evanescent white dot syndrome (MEWDS) is an inflammatory eye disease of the outer retina, retinal pigmented epithelium, choroid presenting with photopsia, loss of vision, and temporal scotoma. The patient was a 31-year-old female with a history of vision loss since 11 days ago (left eye). At presentation, best-corrected Snellen visual acuity was 20/140 in the Snellen chart. We decided to treat her with short time corticosteroid therapy (0.75 mg/kg/day prednisolone which was tapered in 3 weeks) for any possible rapid recovery of vision. The visual acuity of the involved eye was improved to 20/25 a...
