A real-world study of the longitudinal course of itch severity and frequency in adults with atopic dermatitis
AbstractItch is a complex symptom that is both common and burdensome in atopic dermatitis (AD). Yet, little is known about the longitudinal course of itch in AD. A prospective, dermatology practice-based study was performed of adults with AD (n = 463). Patients were assessed at baseline and approximately 6, 12, 18 and 24 months. Itch was assessed using Numeric Rating Scale (NRS) average and worst-itch scores, and frequency of itch in the past week. Outcomes and Factors Associated With Prehospital Treatment of Pediatric Anaphylaxis
Conclusions Prehospital treatment with EPI remains low, and barriers to optimal treatment are more pronounced in children with public insurance. Prehospital treatment with EPI was associated with decreased morbidity including hospitalization and intensive care unit admission.

Rhinology Medicare reimbursements have not been keeping up with inflation
ConclusionMedicare reimbursements are complex, adjusted yearly, and undergo constant federal scrutiny due to the increasing costs of health care. These results suggest that, in terms of real dollars, rhinologic procedures have seen a large gradual decrease in Medicare reimbursement, which is important information for policymakers and surgeons alike.

Recently, the federal government, through the Centers for Medicare and Medicaid Services Innovation (CMMI), was instructed to work toward value-based health care delivery models.1 The CMMI was developed to address these challenges through creativity and innovation.2 The allergy-immunology specialty is small (less than 6000 physicians), but our perspective of our nation's health care delivery system may be instructive for all the medicine as we adapt to these and other disruptive changes in health care.
