The need to link anaphylaxis signs and symptoms with targeted therapeutic strategies
Anaphylaxis is challenging to diagnose and manage for patients, caregivers, and clinicians alike, largely because there is no reference standard diagnostic test. The diagnosis instead relies on a combination of signs and symptoms from up to 5 organ systems, including the mucosal/dermatologic, gastrointestinal, respiratory, cardiovascular, and neurologic systems.1,2 Guidelines emphasize epinephrine as the first-line anaphylaxis therapy, and repeat dosing or initiation of an epinephrine infusion is indicated for refractory cases.

Marginal Zone
Cartootorial: In 1930, LA Wadell reported that Pharaoh Menes of Egypt died in 2641 BCE of insect sting anaphylaxis, a fact that has oft been repeated in the Allergy literature.1,2 While intriguing, this isn't supported by other historians.1 –3 Wadell's claim was based on his own interpretation of hieroglyphs from a purported burial site for Menes, which he said depicted wasps.1,2 In contrast, Manetho, a 3rd century BCE priest and Egyptian historian, wrote extensively about Menes, stating he “was carried off by a hippopotamus and pe rished.”2,3 Modern historians have noted similarities in hieroglyphs used to depict wa...

Evidence-based use of antihistamines for treatment of allergic conditions
Ann Allergy Asthma Immunol. 2023 Jul 28:S1081-1206(23)00524-0. doi: 10.1016/j.anai.2023.07.019. Online ahead of print.ABSTRACTAvailable since the 1940s, H1-antihistamines are mainstay treatments for allergic conditions such as allergic rhinitis and urticaria. They function as inverse agonists that bind to the H1 receptor to inhibit histamine-induced inflammation. The older, first-generation drugs are no longer recommended for patient use due to their well documented negative side effect profile. Evidence has been accumulating to support a newer generation of H1-antihistamines in oral and intranasal formulations, including ...

Food Protein-induced Enterocolitis Syndrome Due to Cuttlefish in a Child with Anaphylaxis to Crustaceans
This report illustrates a rare case of a 6-year-old girl with challenge-proven acute food protein-induced enterocolitis syndrome (FPIES) to cuttlefish (phylum Mollusca, class Cephalopoda), anaphylaxis to crustaceans (phylum Arthropoda), and tolerance to other mollusks, including clams and mussels (phylum Mollusca, class Bivalvia). The association of IgE-mediated food allergy and acute FPIES seen in this case is rare. To our knowledge, this is the first case of FPIES to cuttlefish reported in a child. This challenge highlights the need for further research into the allergens and mechanisms underpinning FPIES at a molecular ...

Anaphylaxis secondary to citric acid allergy in End Stage Renal Disease patient
AbstractDialysis reactions are not uncommon in End Stage Renal Disease (ESRD) patients who are undergoing in-center hemodialysis. Here we report the first case of anaphylaxis related to citric acid solutions used for dialysis machine disinfection and descaling. The patient developed repeated episodes of anaphylactic reactions during hemodialysis only in the inpatient setting leading to respiratory failure needing intubation and anaphylactic shock. This had failed to resolve despite using various combinations of different dialysis machines and dialyzer membranes and only resolved after the elimination of citric acid from th...

Different age groups present different clinics in anaphylaxis with children: one size does not fit all children
This study aimed to determine the clinical features and age-specific patterns of childhood anaphylaxis in a single referral center in Turkey.METHODS: We conducted a retrospective study of anaphylaxis in children aged between 0 and 18 years of age, attending an allergy department in a children's hospital.RESULTS: A total of 95 children diagnosed with anaphylaxis were analyzed. Among all, 35.8% of the first anaphylaxis episodes occurred ininfancy and 57.9% in preschool age. Foods were the most common culprits (57.9%) and followed by drugs (15.8%). Patients with foodinduced anaphylaxis were younger in age (p < 0.001). Food...
